Isosorbide mononitrate
Indications/Uses
Prophylaxis of angina & adjunct in CHF.
Dosage/Direction for Use
Initially 30 mg (given as single 30-mg tab or ½ of 60-mg tab) or 60 mg (given as single tab) once daily. After several days, may be increased to 120 mg (given as single 120-mg tab or as two 60-mg tab) once daily.
Administration
May be taken with or without food: Swallow whole w/ ½ glass liqd, do not chew/crush. Preferably taken in the morning upon arising.
Contraindications
Hypersensitivity or idiosyncratic reactions to isosorbide mononitrate, other nitrates or nitrites.
Special Precautions
Severe hypotension can result from amplification of vasodilatory effects by sildenafil, & may occur, particularly w/ upright posture, even w/ small doses of isosorbide mononitrate. Use w/ caution in patients who may be vol-depleted or who are already hypotensive for whatever reason; severe renal or severe hepatic impairment, hypothyroidism, malnutrition or hypothermia. Not recommended in patients w/ acute MI or CHF. Not to be used in patients w/ hypovolemia, marked anemia, heart failure due to obstruction (including constrictive pericarditis), or raised ICP due to head trauma or cerebral hemorrhage. May aggravate angina caused by hypertrophic cardiomyopathy. Tolerance may occur (long-term exposure to presumably high doses of organic nitrates). Daily headaches sometimes accompany treatment; patients should resist temptation to avoid headaches by altering schedule of treatment. May be associated w/ lightheadedness on standing, especially just after rising from a recumbent or seated position; effect may be more frequent on patients who have consumed alcohol. Pregnancy & lactation.
Adverse Reactions
Flushing of the face, dizziness, tachycardia, throbbing headache. Large doses may cause vomiting, restlessness, blurred vision, hypotension, syncope, & cyanosis (rarely), methemoglobinemia, impairment of respiration & bradycardia.
Drug Interactions
Vasodilating effects may be additive w/ those of other vasodilators & other drugs w/ hypotensive actions, particularly alcohol. Reports of marked symptomatic orthostatic hypotension w/ Ca channel blockers.
