Vancomycin HCl
Indications/Uses
Secondary superficial infections eg, endocarditis, osteomyelitis, arthritis, peritonitis, meningitis, burn & operative wound; pneumonia, sepsis, pulmonary pyosis, empyema thoracis; severe infections which have failed to respond to penicillins or cephalosporins; enterocolitis & antibiotic-associated pseudomembranous colitis caused by Clostridium difficile.
Dosage/Direction for Use
Administer intermittent IV infusion for over a period of at least 60 min. Adult 500 mg every 6 hr or 1 g every 12 hr. Antibiotic associated pseudomembranous colitis produced by C. difficile 500 mg-2 g in 3 or 4 divided doses for 7-10 days, Max daily dose: 2 g. Childn 10 mg/kg every 6 hr. Neonate & young infant Initially 15 mg/kg, followed by 10 mg/kg every 12 hr in the 1st wk of life & every 8 hr thereafter until 1 mth of age.
Contraindications
Hypersensitivity to vancomycin, peptide or aminoglycoside antibiotics. Patient w/ history of shock w/ vancomycin.
Special Precautions
Not to be used in patients w/ auditory disorder caused by peptide & aminoglycoside antibiotics or w/ other auditory disorder. Patients w/ cochlea damage or vestibular organ damage. Use in confirmed susceptibility & min period to prevent the development of drug-resistant bacteria; prolonged use may result in the overgrowth of non-susceptible organism. Discontinue therapy if symptoms including diarrhea, belly ache & pyrexia in treating antibiotic-associated pseudomembranous colitis has not improved w/in 7-10 days. Regularly perform blood levels test, urinalysis & renal function tests. Renal dysfunction eg, acute urinary retention. Monitor leukocyte count in patients who will undergo prolonged therapy or who are receiving concomitant drugs causing neutropenia. Hepatic impairment. Pregnancy. Avoid during lactation. Prematures, neonates & infants who are under developmental stage of kidney. Elderly.
Adverse Reactions
Reversible neutropenia; elevated bilirubin, AST, ALT & ALP; increased serum creatinine or BUN conc; exfoliative dermatitis, bullous dermatitis, SJS & TEN; Rash, flare, facial flushing, Hypotension, wheezing, dyspnea, urticaria, pruritus, rapid infusion may also be associated w/ pain & muscle spasm of the chest & back or red neck or red man syndrome. Maculopapular rash, purpura (purpuric eruption), follicular eruption; jaundice; rash.
Drug Interactions
Concomitant administration w/ anesth agents may enhance anaphylactoid reactions. Concurrent or sequential systemic or topical use w/ other potentially neurotoxic or nephrotoxic drugs eg, amphotericin B, aminoglycosides, bacitracin, polymyxin B, colistin, viomycin, cisplatin, nedaplatin. Concurrent use w/ anticoagulant increases anticoagulant effect.
