Amitriptyline HCl

Indications/Uses

Treatment of depression, nocturnal enuresis, anxiety disorders (eg, panic disorder). Management of neuropathic pain.

Dosage/Direction for Use

Give dose 30 min before bedtime & treatment, including period of gradual w/drawal, should not be continued for >3 mth. Childn >11 yr 25-50 mg, 6-10 yr 10-20 mg.

Overdosage

Administration

May be taken with or without food.

Contraindications

Hypersensitivity. Not to be given concomitantly w/ MAOIs.

Special Precautions

Avoid use in patients w/ heart block, cardiac arrhythmias or in the immediate recovery period after MI. Caution in patients w/ urinary retention, prostatic hyperplasia or chronic constipation; untreated angle-closure glaucoma & phaeochromocytoma; history of epilepsy, hyperthyroidism; CV disease. Suicidal thoughts & behaviour may develop during early treatment for other disorders. May precipitate mania when used for the depressive component of bipolar disorders. May aggravate psychotic symptoms when used for the depressive component of schizophrenia. Do not give to patients receiving MAOIs or for at least 2 wk (3 wk if starting clomipramine or imipramine) after w/drawal. Pregnancy. Avoid use during breast feeding.

Adverse Reactions

Dry mouth, constipation (occasionally leading to paralytic ileus), urinary retention, blurred vision & disturbances in accommodation, increased IOP & hyperthermia; drowsiness; headache, peripheral neuropathy, tremor, ataxia, epileptiform seizures, tinnitus & occasional extrapyramidal symptoms including dysarthria; confusion, hallucinations, or delirium may occur particularly in elderly, & mania or hypomania, & behavioural disturbances (particularly in childn).

Drug Interactions

Concomitant use w/ different antidepressants. Risk of arrhythmias may be increased w/ phenytoin.
