Indications/Uses
Short-term use for hemorrhage or risk of hemorrhage in increased fibrinolysis or fibrinogenolysis. Local fibrinolysis in prostatectomy & bladder surgery; menorrhagia; epistaxis; conisation of the cervix; traumatic hyphema; hereditary angioneurotic edema; management of dental extraction in hemophiliacs.
Dosage/Direction for Use
Local fibrinolysis 15-25 mg/kg (2-3 tab) bid-tid. Prostatectomy 2 tab tid-qid until macroscopic hematuria is no longer present. Menorrhagia 2 tab tid up to 4 days, may be increased w/ very heavy menstrual bleeding. Max: 4 g daily (8 tab). Epistaxis 2 tab tid for 7 days. Conisation of the cervix 3 tab tid. Traumatic hyphema 2-3 tab tid. Dose is based on 25 mg/kg tid. Hereditary angioneurotic edema Intermittently 2-3 tab bid-tid for some days. Hemophilia in the management of dental extractions 2-3 tab every 8 hr. Dose is based on 25 mg/kg. Mild to moderate renal insufficiency serum creatinine 250-500 μmol/L 15 mg/kg daily, 120-249 μmol/L 15 mg/kg bid. Childn 25 mg/kg/dose.
Administration
May be taken with or without food: Swallow whole, do not chew/break.
Contraindications
Hypersensitivity. Active thromboembolic disease. History of venous or arterial thrombosis; convulsions. Fibrinolytic conditions following consumption coagulopathy. Severe renal impairment.
Special Precautions
Risk of mechanical anuria due to formation of ureteral clot. Perform regular eye exam & LFTs in long-term treatment of patients w/ hereditary angioneurotic edema. Not to be used in patients w/ irregular menstrual bleeding. Increased risk of thrombosis in patients receiving OCs. Previous thromboembolic event & family history of thromboembolic disease (eg, thrombophilia). Not recommended in cases of increased fibrinolysis due to disseminated intravascular coagulation. Discontinue treatment in patients who experience visual disturbances. Convulsions. Increased blood levels in patients w/ renal insufficiency. Pregnancy & lactation. Menorrhagic childn <15 yr.
Drug Interactions
May counteract the thrombolytic effect of fibrinolytic prep.
