Vancomycin HCl
Indications/Uses
Serious or severe infections caused by susceptible strains of methicillin-resistant (β-lactam-resistant) staphylococci. Penicillin-allergic patients, for patients who cannot receive or who have failed to respond to other drugs, including penicillins or cephalosporins & for infections caused by vancomycin-susceptible organisms that are resistant to other antimicrobial drugs. Initial therapy when methicillin resistant staphylococci are suspected. Staphylococcal endocarditis. Alone or in combination w/ aminoglycoside for endocarditis caused by S. viridans or S. bovis; endocarditis caused by E. faecalis in combination w/ aminoglycoside. Diphtheroid endocarditis. Treatment or prevent infections that are proven or strongly suspected to be caused by susceptible bacteria. Antibiotic-associated Pseudomembranous colitis produced by C. difficile & for Staphylococcal enterocolitis.
Dosage/Direction for Use
IV Adult 2 g divided either as 500 mg every 6 hr or 1 g every 12 hr administered at no ≥10 mg/min or at least 60 min. Childn 10 mg/kg/dose every 6 hr administered over at least 60 min. Neonate up to 1 mth Initially 15 mg/kg followed by 10 mg/kg every 12 hr in the 1st wk of life & every 8 hr thereafter up to 1 mth administered over 60 min. Mild or moderate renal insufficiency Initially 15 mg/kg.
Contraindications
Special Precautions
Exaggerated hypotension, including shock when administered via rapid bolus. Should be diluted over a period of not <60 min to avoid rapid-infusion-related reactions. Transient or permanent ototoxicity. Adjust dosage in patients w/ renal dysfunction. C. difficile associated diarrhea. Pseudomembranous colitis due to C. difficile. Monitor renal function; leucocyte count. Extravasation; thrombophlebitis. Infusion-related events (including hypotension, flushing, erythema, urticaria & pruritus). Chemical peritonitis in IP route.
Adverse Reactions
Rapid infusion: Anaphylactoid reactions, including hypotension, wheezing, dyspnea, urticaria, or pruritus; flushing of the upper body (''red neck'') or pain & muscle spasm of the chest & back. Increased serum creatinine or BUN conc. Pseudomembranous colitis. Ototoxicity. Neutropenia. Inflammation at the inj site. Anaphylaxis, drug fever, nausea, chills, eosinophilia, rashes including exfoliative dermatitis, linear IgA bullous dermatosis, SJS, TENS & vasculitis.
Drug Interactions
Erythema & histamine-like flushing & anaphylactoid reactions w/ anesth. Potential neurotoxicity &/or nephrotoxicity w/ amphotericin B, aminoglycosides, bacitracin, polymyxin B, colistin, viomycin, or cisplatin.
