Donepezil HCl
Indications/Uses
Symptomatic treatment of mild-moderately severe Alzheimer's disease & vascular dementia (associated w/ CV disease).
Dosage/Direction for Use
Adult & elderly 5 mg/day in the evening just prior to retiring. Maintain dose for at least 1 mth, & may be increased to max of 10 mg/day thereafter.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to donepezil HCl & piperidine derivatives.
Special Precautions
Patients w/ severe Alzheimer's dementia, other types of dementia or memory impairment (eg, age-related cognitive decline); sick sinus syndrome or other supraventricular cardiac conduction conditions eg, SA or AV shock; increased risk for developing ulcers eg, history of ulcer disease (or those receiving NSAIDs); history of asthma or obstructive pulmonary disease. May likely to exaggerate succinylcholine-type muscle relaxation during anesth. May have vagotonic effects on heart rate (eg, bradycardia). Risk of bladder outflow obstruction; generalized convulsions. May exacerbate or induce extrapyramidal symptoms. Not to be used in patients w/ rare hereditary problems of galactose intolerance, lactose deficiency or glucose-galactose malabsorption. Concomitant use w/ inhibitors of acetylcholinesterase, agonists or antagonists of the cholinergic system. Severe hepatic impairment. Pregnancy. Not to be used during lactation. Not recommended for childn & adolescents <18 yr.
Adverse Reactions
Diarrhoea, nausea; headache. Common cold; anorexia; hallucinations, agitation, aggressive behaviour, abnormal dreams & nightmares; syncope, dizziness; vomiting, abdominal disturbance; rash, pruritus; muscle cramps; urinary incontinence; fatigue, pain; accident.
Drug Interactions
Inhibited metabolism w/ CYP3A4 (eg, ketoconazole, itraconazole, erythromycin) & CYP2D6 (eg, quinidine, fluoxetine) inhibitors. Levels may be reduced by enzyme inducers (eg, rifampicin, phenytoin, carbamazepine) & alcohol. May interfere w/ drugs having anticholinergic activity. Potential synergistic activity w/ succinylcholine, other neuro-muscular blocking agents or cholinergic agonists or β-blocking agents w/ effects on cardiac conduction.
