Indications/Uses
Bipolar disorder including manic or depressive episodes associated w/ bipolar disorder; preventing recurrence in maintenance treatment of bipolar disorder (manic, mixed or depressive episode) as monotherapy or in combination w/ mood stabilizers. Seroquel Acute & chronic psychoses including schizophrenia. Seroquel XR Schizophrenia. Preventing relapse in stable schizophrenic patients maintained on quetiapine. Major depressive disorder. Preventing relapse in stable major depressive disorder patients maintained on quetiapine. Generalised anxiety disorder. Preventing relapse in stable patients w/ generalised anxiety disorder maintained on quetiapine.
Dosage/Direction for Use
Seroquel Adult Acute & chronic psychoses including schizophrenia Total daily dose for 1st 4 days of therapy: 50 mg (day 1), 100 mg (day 2), 200 mg (day 3), 300 mg (day 4). From day 4 onwards, titrate to 300-450 mg/day. May be adjusted w/in the range of 150-750 mg/day. Manic episodes associated w/ bipolar disorder Total daily dose for 1st 4 days of therapy: 100 mg (day 1), 200 mg (day 2), 300 mg (day 3), 400 mg (day 4); up to 800 mg/day by day 6 in increments of not >200 mg/day. Dose may be adjusted w/in the range of 200-800 mg/day depending on clinical response & tolerability of the patient. Usual effective dose: 400-800 mg/day. Depressive episodes associated w/ bipolar disorder 50 mg (day 1), 100 mg (day 2), 200 mg (day 3) & 300 mg (day 4). Dose may be titrated to 400 mg on day 5 & up to 600 mg by day 8. Preventing recurrence in maintenance treatment of bipolar disorder Patients who have responded to quetiapine in combination therapy to a mood stabilizer (lithium or valproate) for acute treatment of bipolar disorder Continue therapy at the same dose. Can be re-adjusted w/in the range of 400-800 mg/day. Patients who have responded to quetiapine for acute treatment of bipolar disorder Continue on therapy at the same dosing regimen. Can be re-adjusted w/in the range of 300-800 mg/day. Childn & adolescent 13-17 yr Schizophrenia Total daily dose for the initial 5 days of therapy: 50 mg (day 1), 100 mg (day 2), 200 mg (day 3), 300 mg (day 4), 400 mg (day 5). After day 5, dose may be adjusted w/in the effective dose range 400-800 mg/day depending on clinical response & tolerability of the patient; increments of not >100 mg/day. Treatment of manic episodes associated w/ bipolar disorder in childn & adolescents 10-17 yr Total daily dose for the initial 5 days of therapy: 50 mg (day 1), 100 mg (day 2), 200 mg (day 3), 300 mg (day 4), 400 mg (day 5). After day 5, dose may be adjusted w/in the range 400-600 mg/day depending on clinical response & tolerability of the patient; increments of not >100 mg/day. Hepatic impairment 25 mg/day, increased daily in increments of 25-50 mg/day depending on the clinical response & tolerability of the patient. Seroquel XR Adult Schizophrenia & manic episodes associated w/ bipolar disorder Daily dose: 300 mg (day 1), 600 mg (day 2) & up to 800 mg after day 2. May be adjusted w/in range of 400-800 mg/day depending on clinical response & tolerability of the patient. Depressive episodes associated w/ bipolar disorder 50 mg (day 1), 100 mg (day 2), 200 mg (day 3) & 300 mg (day 4). Dose may be titrated to 400 mg (day 5) up to 600 mg (day 8). Dose may be increased in increments of 50 mg/day depending on clinical response & tolerability. Elderly w/ major depressive disorder/generalized anxiety disorder 50 mg (days 1-3), may increase to 100 mg (day 4), 150 mg (day 8) & up to 300 mg depending on clinical response & tolerability. Hepatic impairment 50 mg/day, increased in increments of 50 mg/day to an effective dose, depending on the clinical response & tolerability of the patient.
Administration
May be taken with or without food: Avoid taking w/ high-fat meals. Seroquel XR tab: Swallow whole, do not split/chew/crush.
Special Precautions
Monitor suicide/suicidal thoughts or clinical worsening; neutropenia & agranulocytosis; increase in blood glucose & hyperglycemia; changes in lipids & metabolic factors; pancreatitis; history of or at risk for sleep apnea & patients receiving concomitant CNS depressants; dysphagia & aspiration; constipation & intestinal obstruction; tardive dyskinesia & extrapyramidal symptoms; neuroleptic malignant syndrome; QT prolongation; cardiomyopathy & myocarditis. Patients w/ known CV or cerebrovascular disease or other conditions predisposing to hypotension; history of seizures; patients receiving medications w/ anticholinergic (muscarinic) effects; current diagnosis or prior history of urinary retention, clinically significant prostatic hypertrophy, intestinal obstruction or related conditions, increased IOP or narrow angle glaucoma. Concomitant use w/ hepatic enzyme inducers eg, carbamazepine & potent CYP3A4 inhibitors (eg, azole antifungals, macrolides & PIs). Avoid abrupt w/drawal. Alcohol or drug abuse. May affect ability to drive or operate machinery. Pregnancy & lactation. Childn & adolescents (10-17 yr). Elderly w/ dementia.
Adverse Reactions
Somnolence, dizziness, dry mouth, w/drawal (discontinuation) symptoms, elevations in serum triglyceride levels & total cholesterol (predominantly LDL cholesterol), decreases in HDL cholesterol, wt gain, decreased Hb & extrapyramidal symptoms.
Drug Interactions
Other centrally acting drugs & alcohol. Drugs known to cause electrolyte imbalance or increase QT interval. Medications w/ anticholinergic (muscarinic) effects. Increased clearance w/ thioridazine, phenytoin, barbiturates, rifampicin. Increased Cmax & AUC w/ a corresponding decrease in mean oral clearance w/ ketoconazole. False +ve results in enzyme immunoassays for methadone & TCAs.
