Meropenem trihydrate
Indications/Uses
Skin & skin structure, intra-abdominal, lower resp tract & gynecological infections; bacterial meningitis; complicated UTI; septicemia. Empiric treatment of patients w/ febrile neutropenia as monotherapy or combination therapy w/ other antimicrobial drugs.
Dosage/Direction for Use
IV Administer bolus inj 5-20 mL over approx 3-5 min or infusion over approx 15-30 min. Adult 500 mg to 1 g every 8 hr. Skin & skin structure infections, gynecological infections & complicated UTI 500 mg every 8 hr. Intra-abdominal infections, septicemia & febrile episodes in neutropenic patients 1 g every 8 hr. Bacterial meningitis 2 g every 8 hr. Lower resp tract infections: community-acquired pneumonia (CAP) 500 mg every 8 hr, healthcare-associated pneumonia 1 g every 8 hr, cystic fibrosis up to 2 g every 8 hr. Childn >50 kg Adult dose for 7-14 days. Childn 3 mth to 12 yr weighing 50 kg 10-40 mg/kg every 8 hr. Complicated UTI 10 mg/kg/dose every 8 hr. Complicated intra-abdominal infection & febrile episodes in neutropenic patient 20 mg/kg/dose every 8 hr. Skin & skin structure infections & CAP 10-20 mg/kg/dose every 8 hr. Cystic fibrosis & meningitis 40 mg/kg/dose every 8 hr. Adult Renal impairment CrCl 26-50 mL/min Usual dose every 12 hr, 10-25 mL/min ½ usual dose every 12 hr, <10 mL/min ½ usual dose every 24 hr.
Contraindications
Hypersensitivity to meropenem, drugs in the same class or other β-lactams.
Special Precautions
Hypersensitivity; anaphylactic reactions. Seizure & other CNS experience. Clostridium difficile-associated diarrhea; risk of development of drug-resistant bacteria; overgrowth of non-susceptible organisms. Patients w/ restricted Na diet. Cross-allergenicity among β-lactams including penicillins, cephalosporins or other β-lactams. Concomitant use w/ valproic acid. +ve direct or indirect Coombs' test. Renal & hepatic impairment. Avoid performing tasks requiring complete mental alertness eg, driving & operating machinery. Pregnancy & lactation. Childn <3 mth. Elderly.
Adverse Reactions
GI effects (diarrhea, nausea, vomiting, constipation), local reactions (pain, edema & inflammation at inj site, phlebitis/thrombophlebitis), headache, anemia, rash, pruritus, sepsis, apnea, shock, glossitis, oral candidiasis & thrombocytosis. Childn: Rash mostly diaper area moniliasis, oral moniliasis & glossitis.
Drug Interactions
Synergistic antibacterial effects w/ aminoglycosides. May augment anticoagulant effect of warfarin. Increased plasma conc w/ probenecid. Reduction in acid conc of valproic acid.
