Indications/Uses
Hypovolemic shock associated w/ blood loss, trauma & surgical procedures. Severe burns (>15% BSA) after 1st 24 hr, if hypoproteinaemia develops &/or to maintain plasma vol. Hypoproteinemia caused by loss of plasma proteins. Used to maintain CV function following removal of large vol of ascitic fluid in patients suffering from ascites. Adjunct in patients who are undergoing long-term hemodialysis & are susceptible to shock & hypotension, or in dialysis patients who are hypovolemic & may not tolerate large vol of crystalloid infusion as treatment for shock or hypotension. For plasma vol replacement & restoration of colloid osmotic pressure. As exchange fluid in therapeutic plasmapheresis. Neonatal hyperbilirubinemia.
Dosage/Direction for Use
IV May be given w/o dilution or diluted w/ normal saline or glucose 5% before administration. Amount of soln will depend upon clinical condition of patient & response to treatment. Acute hypovolemic shock Adult Initially 25 g. Childn Up to about 1 g/kg. Hypoproteinemia Max: 2 g/kg daily. Neonatal hyperbilirubinemia 1 g/kg before exchange transfusion. Adjust rate of infusion according to indication & patient response, but in general, suggested rates of infusions are up to 1-2 mL/min (20% soln). Plasmapheresis Adjust infusion rate according to rate of removal.
Contraindications
Patients w/ history of allergic reactions to albumin prep. Patients w/ severe anemia or cardiac failure in presence of normal or increased intravascular vol.
Special Precautions
Not to be used for parenteral nutrition. Do not use soln if turbid in appearance or if there is sediment in bottle. Do not use content >4 hr after container has been penetrated. Discard unused portion. Administer w/ caution to patients w/ low cardiac reserve. Rapid infusion may cause vascular overload w/ resultant pulmonary edema. Monitor patients closely for signs of increased venous pressure. Patients w/ marked dehydration require administration of additional fluids. May be administered w/ usual dextrose & saline IV soln. Never infuse soln containing protein hydrolysates or alcohol through same administration set since combinations may cause precipitation of proteins. Pregnancy. Childn.
Adverse Reactions
Allergic or pyrogenic reactions eg, fever & chills. Rash, nausea, vomiting, tachycardia, hypotension & increased salivation. Vascular overload w/ resultant pulmonary edema if administered rapidly.
