Pneumococcal polysaccharide conjugate vaccine, 13-valent (adsorbed)
Indications/Uses
Prevention of invasive diseases caused by 13 serotypes (1, 3, 4, 5, 6A, 6B, 7F, 9V, 14, 18C, 19A, 19F & 23F) of Strep pneumoniae in infants & childn 6 wk-5 yr (before 6th birthday).
Dosage/Direction for Use
IM 1 dose of 0.5 mL inj in anterolateral thigh in infants & in deltoid muscle of upper arm in toddlers & childn. Infant 2 mth Vaccination schedule: 1st dose at 2 mth (dose may be given as early as 6 wk w/ dosing interval of 8 wk); 2nd dose at 4 mth & 3rd dose at 6 mth (dosing interval of 8 wk); 4th dose at approx 12-15 mth & at least 2 mth after 3rd dose. Infant 3 mth Vaccination schedule: 1st dose at 3 mth, 2nd dose at 4 mth & 3rd dose at 5 mth (dosing interval of 4 wk); 4th dose at approx 12-15 mth & at least 2 mth after 3rd dose. Unvaccinated childn 7 mth-5 yr 7-11 mth at 1st dose: 3 doses w/ 1st 2 doses at least 2 mth apart, 3rd dose after 1 yr birthday, separated from 2nd dose by at least 2 mth. 12-23 mth at 1st dose: 2 doses at least 2 mth apart. 24 mth-5 yr (prior to 6th birthday) at 1st dose: Single dose.
Contraindications
Hypersensitivity to any component of pneumococcal polysaccharide conjugate vaccine, or tetanus toxoid.
Special Precautions
Do not vaccinate via IV route or by gluteal IM inj, & ensure syringe needle is not puncturing blood vessels during inj. Postpone vaccination in case of fever, acute diseases, & acute attack of chronic diseases. Appropriate monitoring & medical care & rescue measures should be readily available in case of occurrence of rare hypersensitivity reactions during vaccination. Patients w/ thrombocytopenia, any coagulopathy or those who are receiving anticoagulant treatment. Immunocompromised individuals (eg, malignancy or nephrotic syndrome). Tetanus toxoid contained in the vaccine is not a replacement for routine immunization of tetanus vaccine or tetanus-containing vaccine. Not guaranteed that all recipients can be protected from any diseases caused by Strep pneumoniae. Pregnancy & lactation. Monitor preterm infants for potential risk of apnea during primary series. Consider at least 48 hr of monitoring for preterm infants under hospitalization (gestational age ≤30 wk at birth) vaccinated w/ PCV13-TT according to recommended immunization schedule. Not a replacement of 23-valent pneumococcal polysaccharide vaccine in childn ≥24 mth w/ conditions eg, sickle cell disease, asplenia, HIV infection, chronic illness, or those who are immunocompromised.
Adverse Reactions
Fever, diarrhea; inj site redness. Crying, cough, nausea/vomiting, fatigue/somnolence, allergic reaction; inj site swelling, pain & induration.
