Oxaliplatin

Indications/Uses

Metastatic cancer of the colon or rectum as 1st-line therapy in combination w/ fluorouracil & leucovorin. Adjuvant treatment of stage III colon cancer patients who have undergone complete resection of the primary tumor.

Dosage/Direction for Use

Previously treated metastatic colorectal cancer Combination therapy: 2-day regimen: 85 mg/m² IV infusion on day 1 + fluorouracil & leucovorin. May be repeated at 2-wk interval. Previously untreated & previously treated patient w/ advanced colorectal cancer Given every 2 wk. Day 1: Oxaliplatin 85 mg/m² IV infusion in 250-500 mL D₅W & leucovorin 200 mg/m² IV infusion in D₅W both given over 120 min at the same time in separate bags using a Y-line, followed by 5-FU 400 mg/m² IV bolus given over 2-4 min, followed by 5-FU 600 mg/m² IV infusion in 500 mL D₅W as 22-hr continuous infusion. Day 2: Leucovorin 200 mg/m² IV infusion over 120 min, followed by 5-FU 400 mg/m² IV bolus over 2-4 min, followed by 5-FU 600 mg/m² IV infusion in 500 mL D₅W as 22-hr continuous infusion.

Special Precautions

Patients w/ preexisting renal impairment. Perform CBC, Hb counts. Reduces blood cells for clotting, lowers resistance to infection. Live vaccines. Avoid drinking cold drink & using ice during treatment. May impair ability to drive or operate machinery. Pregnancy & lactation.

Adverse Reactions

Nausea, vomiting, diarrhea; tingling, numbness, neuropathy; reversible sensitivity to cold, tightness or spasm in throat; myelosuppression, neutropenia; fever & infection, risk of bruising & bleeding; anaphylaxis, breathing difficulty, drop in BP, sweating, flushing, dizziness, headache, fast heart beat.
