Indications/Uses

Secondary treatment of acute hypotension or shock according to different diseases (shock due to MI & septicemia caused by shock, anaphylactic shock, hypotension or shock, according to decreased circulating blood vol, acute hypotension during anesth); cardiac arrest.

Dosage/Direction for Use

Acute hypotension or shock Increase or decrease properly by diluting 8 mg (norepinephrine as 4 mg) to 5% glucose inj liqd or 1,000 mL of 5% glucose saline inj while observing the reaction by IV infusion at 2-3 mL/min. Maintenance: 0.5-1 mL/min. Cardiac arrest Administer IV inj for case of acute hypotension or shock after retaining valid heart rate & ventilation while cardiac resuscitation.

Overdosage

Contraindications

Low BP due to lack of blood vol, high BP, hyperthyroidism, pheochromocytoma, pulmonary heart disease, severe arrhythmia. Patients w/ anesthetized fingers, toes, & nose. Renal dysfunction. Pregnancy.

Special Precautions

Hypersensitivity. Patients w/ cocaine poisoning; ventricular tachycardia; atherosclerosis; heart disease; bradycardia; coronary, mesenteric, peripheral vascular thrombosis; hypotension due to MI; Prinzmetal angina. May cause an excessive increase in BP & acute pulmonary edema action, arrhythmia, cardiac arrest. Check the BP & administration rate while injecting. Use α blocker (phentolamine) if excessive rise in BP. Avoid administering for pregnant or possible pregnant women. Lactation. Elderly.

Adverse Reactions

Ischemic injury due to potent vasoconstrictor action & tissue hypoxia. Cardiogenic shock, arrhythmia, bradycardia, stress cardiomyopathy; anxiety, transient headache; resp difficulty; extravasation necrosis at inj site.

Drug Interactions

Increased cardiac autonomic irritability & risk of producing ventricular tachycardia or fibrillation w/ cyclopropane & halothane anesth. Severe, prolonged HTN w/ MAOIs or antidepressants of the triptyline or imipramine types.
