Metformin HCl
Indications/Uses
Diet-failed, NIDDM patients, especially if overwt, as monotherapy or as initial therapy w/ sulphonylurea.
Dosage/Direction for Use
NIDDM Initially 500 mg bid or tid, or 850 mg once daily or bid, gradually increased if necessary to a max of 2 or 3 g daily.
Administration
Should be taken with food: Take w/ or after meals.
Contraindications
Diabetic coma, ketoacidosis, impaired renal function, chronic liver disease. Cardiac failure, recent MI, alcoholism, tissue hypoxia, lactic acidosis. Severe infections, trauma, surgery, dehydration.
Special Precautions
Not to be used in IDDM; patients w/ heart failure, dehydration, acute or chronic alcoholism, or any other condition, likely to predispose to lactic acidosis. Not to be treated w/ metformin when patient plans to become pregnant & during pregnancy. Not recommended during breastfeeding.
Adverse Reactions
GI adverse effects w/ anorexia, nausea, & vomiting; impaired absorption of various substances including vit B12. Metallic taste & wt loss. Hypoglycemia. Lactic acidosis (particularly those w/ renal impairment).
Drug Interactions
Increased risk of hypoglycaemia w/ other drugs that lower blood glucose conc. Reduced effect w/ drugs that increase blood glucose conc. May increase risk of lactic acidosis as well as hypoglycaemia w/ alcohol. Care should be taken if given concomitantly w/ drugs that may impair renal function.
