Indications/Uses
Life-threatening infections due to susceptible gm +ve organisms which cannot be treated by penicillins & cephalosporins. Severe Staph infections in patients who cannot receive or who have failed to respond to the penicillins & cephalosporins or who have infections w/ Staph that are resistant to other antibiotics. Endocarditis & prophylaxis against endocarditis in patients at risk from dental or surgical procedures. Osteomyelitis, pneumonia, septicemia & soft tissue infections.
Dosage/Direction for Use
IV infusion Adult 500 mg every 6 hr or 1 g every 12 hr over 60 min. Childn >1 mth 10 mg/kg per dose every 6 hr over 60 min. Infant Initially 15 mg/kg, followed by 10 mg/kg every 12 hr for neonates in 1st wk of life & thereafter every 8 hr up to 1 mth over 60 min.
Contraindications
Special Precautions
Hearing loss. Avoid long-term therapy. Colitis, severe diarrhea. Thrombophlebitis may occur w/ high conc IV administration. Avoid rapid infusion as may cause hypotension. Do not mix w/ drug w/ alkaline soln & w/ β-lactam antibiotics. Impaired vestibular organ or cochlea. Renal impairment. Pregnancy & lactation. Elderly. Premature infant, neonate & toddler.
Adverse Reactions
Red man syndrome. Inj site pain & swelling or blood vessel inflammation. Dizziness, tinnitus, hearing loss. Increased bilirubin, AST, ALT & ALP. Increase BUN & creatinine. Exfoliative dermatitis, bullous dermatitis, Stevens-Johnson syndrome & toxic epidermal necrolysis. Rash, erythema, flushing, hypotension, wheezing, dyspnea, urticaria & pruritus. Pseudomembranous colitis.
Drug Interactions
Increased risk of renal or hearing impairment w/ ethacrynic acid, aminoglycosides, bacitracin, polymixin B or colistin, viomycin, amphotericin B, cisplatin or neuromuscular blockers. Increased frequency of anaphylactoid reaction w/ anesth agents. May increase adverse reactions w/ potentially neurotoxic drugs eg, amphotericin B, aminoglycosides, bacitracin, polymyxin B, colistin, viomycin & platinum antitumor drugs (cisplatin, nedaplatin). Increased anticoagulant activity w/ anticoagulant.
