Indications/Uses
Treatment of infections caused by single or multiple susceptible bacteria & as empiric therapy prior to identification of causative organisms: Lower resp tract infections; UTI including complicated infections; intra-abdominal infections; gynaecological infections including postpartum infections; skin & skin structure infections; septicaemia; meningitis. Polymicrobial infections. Cystic fibrosis & chronic lower resp tract infections, either as monotherapy or in combination w/ other antibacterial agents.
Dosage/Direction for Use
IV Administer as bolus over approx 5 min or by infusion over approx 15-30 min. Adult 1.5-6 g daily in 3 divided doses. Usual dose: 500 mg to 1 g every 8 hr depending on type & severity of infection. Febrile episode in neutropenic patient 1 g every 8 hr. Meningitis/cystic fibrosis 2 g every 8 hr. Known or suspected Pseudomonas aeruginosa infection At least 1 g every 8 hr. Max: 6 g daily in 3 divided doses. Infant & childn >3 mth up to 12 yr 10-40 mg/kg every 8 hr. Childn >50 kg Use adult dose. Febrile episode in neutropenic patient 20 mg/kg every 8 hr. Meningitis/cystic fibrosis 40 mg/kg every 8 hr. Known or suspected Pseudomonas aeruginosa infection At least 20 mg/kg every 8 hr. Max: 120 mg/kg daily in 3 divided doses. Adult w/ renal impairment CrCl 26-50 mL/min 1 u dose every 12 hr, 10-25 mL/min ½ u dose every 12 hr, <10 mL/min ½ u dose every 24 hr.
Contraindications
Special Precautions
History of hypersensitivity to β-lactam antibiotics. Discontinue treatment if allergic reaction occurs. Overgrowth of non-susceptible organisms w/ prolonged use. Consider pseudomembranous colitis in patients who present w/ diarrhea subsequent to antibiotics administration. No experience w/ peritoneal dialysis & in childn w/ renal impairment. Pregnancy & lactation.
Adverse Reactions
Pains, inflammation, thrombophlebitis at inj site; urticaria, rash, pruritus, erythema multiforme, TEN & SJS; nausea, vomiting, abdominal pain, diarrhoea, pseudomembranous colitis; increased eosinophils, reversible thrombocythaemia, thrombocytopenia, leucopenia, neutropenia, agranulocytosis, +ve direct or indirect Coombs' test, shortened partial thromboplastin; increased transaminases, alkaline phosphatase, LDH, & bilirubin; paraesthesia, headache, seizures; rarely, systemic allergic reaction (hypersensitivity) may occur following administration, angioedema & anaphylaxis.
Drug Interactions
Increased plasma conc w/ probenecid.
