Contents
Per 40/12.5 mg tab Telmisartan 40 mg, hydrochlorothiazide 12.5 mg. Per 80/12.5 mg tab Telmisartan 80 mg, hydrochlorothiazide 12.5 mg. Per 80/25 mg tab Telmisartan 80 mg, hydrochlorothiazide 25 mg
Indications/Uses
Essential HTN. Adult whose BP is not adequately controlled on telmisartan alone.
Dosage/Direction for Use
Patients whose BP is not adequately controlled by telmisartan alone 1 tab (either 40 mg/12.5 mg, 80 mg/12.5 mg or 80 mg/25 mg) once daily. Mild to moderate hepatic impairment Should not exceed 40 mg/12.5 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to other sulphonamide-derived substance. Cholestasis & biliary obstructive disorders. Refractory hypokalemia, hypercalcemia. Severe hepatic & renal impairment (CrCl <30 mL/min). Patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2). Pregnancy (2nd & 3rd trimester).
Special Precautions
Hypersensitivity. Risk of severe hypotension & renal insufficiency. Hepatic impairment. Must not be used in patients w/ severe renal impairment (CrCl <30 mL/min). Intravascular hypovolaemia. Dual blockade of the renin-angiotensin-aldosterone system. Patients w/ severe CHF or underlying renal disease including renal artery stenosis. Primary aldosteronism. Aortic or mitral stenosis or obstructive hypertrophic cardiomyopathy. May impair glucose tolerance. Electrolyte imbalance. Hypo/hyperkalemia. Hyponatremia & hyperchloremic alkalosis. Hypercalcemia. Fructose intolerance &/or w/ galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption. Black hypertensive patients. MI or stroke. Photosensitivity reactions. Acute myopia & angle-closure glaucoma. May influence ability to drive & use machines. Pregnancy & lactation.
Drug Interactions
Lithium. Other kaliuretic diuretics, laxatives, corticosteroids, ACTH, amphotericin, carbenoxolone, penicillin G Na, salicylic acid derivatives. May increase K levels or induce hyperkalemia w/ ACE inhibitors, K-sparing diuretics, K supplements, salt substitutes containing K, cyclosporin, heparin Na. Serum K disturbances w/ class Ia antiarrhythmics, class III antiarrhythmics, antipsychotics (eg, thioridazine, chlorpromazine, levomepromazine, trifluoperazine, cyamemazine, sulpiride, sultopride, amisulpride, tiapride, pimozide, haloperidol, droperidol); others (eg, bepridil, cisapride, diphemanil, erythromycin IV, halofantrine, pentamidine, sparfloxacin, terfenadine, vincamine IV). Thiazide-induced hypokalemia w/ digitalis glycosides. Increase in digoxin peak plasma conc. May increase hypotensive effect or other antihypertensive agents. Oral antidiabetics & insulin. Risk of lactic acidosis w/ metformin. Cholestyramine & colestipol resins. NSAIDs (ie, ASA, COX-2 inhibitors & non-selective NSAIDs). Decrease noradrenaline effects. Tubocurarine. May raise level of serum uric acid of probenecid, sulfinpyrazone & allopurinol. May increase Ca level of Ca salts. May enhanced hyperglycemic effect of β-blocker & diazoxide. Atropine, biperiden. Amantadine. May reduce renal excretion & potentiation of myelosuppressive effects of cyclophosphamide, MTX; baclofen, amifostine; may aggravate orthostatic hypotension w/ alcohol, barbiturates, narcotics or antidepressants.
