Moderate to severe Alzheimer's disease.

Dosage/Direction for Use

FC tab Starting dose: 5 mg. Max: 20 mg. Dose should be increased in 5 mg increments over the 1st 4 wk of treatment. Doses are taken once daily. Maintenance dose: 20 mg/day. Oral soln Adult Max: 20 mg daily. Upward titration of 5 mg/wk over the 1st 3 wks [wk 1: 1 pump actuation (equiv to 5 mg); wk 2: 2 pump actuations (equiv to 10 mg), wk 3: 3 pump actuations (equiv to 15 mg); wk 4 & beyond: 4 pump actuations (equiv to 20 mg)]. Maintenance dose: 20 mg/day.

Special Precautions

SJS & acute generalized exanthematous pustulosis. Patients w/ seizure; renal tubular acidosis & severe UTI; serious comorbidities. Close supervision in patients w/ recent MI, uncompensated CHF (NYHA III-IV) or uncontrolled HTN. Hepatic & renal impairment. May impair driving performance & compromises ability to use machines. Not to be given in pregnant women. Lactation. Childn <18 yr.

Adverse Reactions

Dizziness, confusion, headache, constipation, cough, HTN, back pain/pain, hallucination, somnolence, vomiting, dyspnea & fatigue.

Drug Interactions

Increased risk of CNS toxicity w/ NMDA eg, amantadine, ketamine & dextromethorphan. Enhanced effect of dopaminergic agonists eg, bromocriptine & anticholinergics. Reduced effect of barbiturates & neuroleptics. Modified antispasmodic effect w/ dantrolene or baclofen. Enhanced oral anticoagulant effect w/ warfarin. Increased plasma levels of cimetidine, ranitidine, procainamide, quinidine, quinine & nicotine. Reduced plasma levels of diuretics eg, hydrochlorothiazide, triamterene. Reduced effect of primidone. Enhanced effect of selegiline.
