Clobetasol propionate
Indications/Uses
Short-term treatment in adult, elderly & childn >1 yr of more resistant inflammatory & pruritic manifestations of steroid-responsive dermatoses unresponsive to less potent corticosteroids including psoriasis (excluding widespread plaque psoriasis), recalcitrant dermatoses, lichen planus, discoid lupus erythematosus & other skin conditions not responding satisfactorily to less potent steroids.
Dosage/Direction for Use
Apply thinly & gently rub in using only enough to cover entire affected area once daily or bid until improvement occurs. Repeated short courses may be used to control exacerbations. Duration: Not to be continued for >4 wk. Limit to 5 days in childn & infant & in application to face. Max: Not to exceed 50 g wkly. Recalcitrant dermatoses Patient who frequently relapses May consider intermittent dosing (once daily, twice wkly) once acute episode has been treated effectively w/ continuous course of topical corticosteroid.
Contraindications
Hypersensitivity. Not to be used to treat untreated cutaneous infections, rosacea, acne vulgaris, pruritus w/o inflammation, perianal & genital pruritus, perioral dermatitis. Dermatoses in childn <1 yr including dermatitis & nappy eruptions.
Special Precautions
Local hypersensitivity to other corticosteroids. Gradually w/draw drug if manifestations of hypercortisolism (Cushing's syndrome) & reversible hypothalamic-pituitary-adrenal axis suppression are observed. Do not smoke or go near naked flames since cream contains paraffin. Osteonecrosis serious infections (including necrotizing fasciitis) & systemic immunosuppression (sometimes resulting in reversible Kaposi's sarcoma lesions) in long-term use beyond recommended doses. Increased risk of systemic effects. Risk of infection w/ occlusion. Concomitant infection. Visual disturbances. Patients w/ psoriasis; chronic leg ulcers. Application to face & eyelids. Avoid abrupt w/drawal of treatment. Contains propylene glycol which may cause skin irritation & cetostearyl alcohol which may cause local skin reactions eg, contact dermatitis. Pregnancy & lactation. Not to be applied to breasts to avoid accidental ingestion by infant. Avoid long-term continuous therapy in infants & childn <12 yr as adrenal suppression may occur.
Adverse Reactions
Pruritus, local skin burning/pain.
Drug Interactions
Inhibited metabolism leading to increased systemic exposure w/ CYP3A4 inhibitors eg, ritonavir & itraconazole.
