Indications/Uses
Treatment of neuropathic pain in adults, including neuropathic pain associated w/ spinal cord injury. Treatment of generalized anxiety disorder (GAD) in adults. Management of fibromyalgia. Adjunctive therapy in adults w/ partial seizures, w/ or w/o secondary generalization.
Dosage/Direction for Use
Neuropathic pain Initially 150 mg daily, may be increased to 300 mg daily after an interval of 3-7 days, & if needed, to max 600 mg daily after an additional 7-day interval. Fibromyalgia Dose range: 300-450 mg daily given in 2 divided doses (some patients may derive additional benefit at 600 mg daily). Initially 75 mg bid, may be increased to 150 mg bid w/in 1 wk. Patients who do not experience sufficient benefit may be further increased to 225 mg bid. If needed, dose may be increased to max 600 mg daily after an additional wk. Epilepsy Initially 150 mg daily, may be increased to 300 mg daily after 1 wk. Max: 600 mg daily may be achieved after an additional wk. GAD Dose range: 150-600 mg daily given in 2 or 3 divided doses. Initially 150 mg daily, may be increased to 300 mg daily after 1 wk. Following an additional wk, dose may be increased to 450 mg daily. Max: 600 mg daily may be achieved after an additional wk. Patient w/ renal impairment CrCl ≥60 mL/min Initially 150 mg daily in 2 or 3 divided doses. Max: 600 mg daily, ≥30-<60 mL/min Initially 75 mg daily in 2 or 3 divided doses. Max: 300 mg daily, ≥15-<30 mL/min Initially 25-50 mg daily as a single dose or in 2 divided doses. Max: 150 mg daily, <15 mL/min Initially 25 mg daily as a single dose. Max: 75 mg daily. Supplementary dosage following hemodialysis Initially 25 mg daily as a single dose, given immediately following every 4-hr hemodialysis. Max: 100 mg daily.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Discontinue immediately if symptoms of angioedema (eg, facial, perioral, or upper airway swelling) occur. Adjust hypoglycemic medications in diabetic patients who gain wt during treatment. Treatment has been associated w/ dizziness & somnolence which could increase the occurrence of accidental injury (fall) in the elderly. Reports of loss of consciousness, confusion, & mental impairment; transient visual blurring & other changes in visual acuity. W/drawal symptoms after discontinuation of short- & long-term treatment have been observed. Caution when considering use in patients w/ current substance abuse or history of substance abuse who are at higher risk for pregabalin abuse. Observe for signs of misuse, abuse or dependence (eg, development of tolerance, dose escalation, drug-seeking behavior). Caution in patients w/ severe CHF. Caution when concomitantly used w/ opioids. Should not be taken by patients w/ galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. May cause dizziness & somnolence & therefore may influence the ability to drive or use machines. Women of childbearing potential must use effective contraception. May cause major birth defects in the unborn child w/ use during 1st trimester of pregnancy. Should not be used during pregnancy unless benefit to the mother clearly outweighs potential risk to the fetus. Not recommended during lactation. Not recommended in childn <12 yr & adolescents (12-17 yr). Elderly >65 yr may require dose reduction due to decreased renal function.
Adverse Reactions
Dizziness, somnolence. Nasopharyngitis; increased appetite; euphoric mood, confusion, irritability, depression, disorientation, insomnia, decreased libido; ataxia, abnormal coordination, tremor, dysarthria, amnesia, memory impairment, disturbance in attention, paresthesia, hypoesthesia, sedation, balance disorder, lethargy; blurred vision, diplopia; vertigo; vomiting, constipation, flatulence, abdominal distension, dry mouth; muscle cramp, arthralgia, back pain, limb pain, cervical spasm; peripheral edema, edema, abnormal gait, fall, feeling drunk & abnormal, fatigue; increased wt. Headache. Nausea, diarrhea.
Drug Interactions
May potentiate effects of ethanol & lorazepam. Additive in the impairment of cognitive & gross motor function caused by oxycodone. Reports of resp failure, coma & deaths w/ other CNS depressants. Reports of events related to reduced lower GIT function (eg, intestinal obstruction, paralytic ileus, constipation) w/ potentially constipation-producing medications eg, opioid analgesics. Risk of CNS depression w/ opioids.
