Sitagliptin phosphate monohydrate

Indications/Uses

Adjunct to diet & exercise to improve glycemic control in adults w/ type 2 DM.

Dosage/Direction for Use

Monotherapy or combination therapy w/ metformin, sulfonylurea, insulin (w/ or w/o metformin), PPARγ agonist (eg, thiazolidinedione), metformin + sulfonylurea or metformin + PPARγ agonist 100 mg once daily. Patient w/ moderate renal insufficiency (CrCl ≥30 to <50 mL/min) 50 mg once daily. Patient w/ severe renal insufficiency (CrCl <30 mL/min) or w/ ESRD requiring hemodialysis or peritoneal dialysis 25 mg once daily.

Overdosage

Administration

May be taken with or without food.

Contraindications

Hypersensitivity. Type 1 DM, diabetic ketoacidosis.

Special Precautions

Discontinue use if hypersensitivity reactions or pancreatitis is suspected. Do not use in patients w/ type 1 DM or for the treatment of diabetic ketoacidosis. Hypoglycemia in combination w/ sulfonylurea or insulin. Assess renal function prior to initiation & periodically thereafter. Renal insufficiency. Not recommended for use during pregnancy. Do not use during lactation. Childn <18 yr.

Adverse Reactions

Hypoglycemia; abdominal pain, nausea, vomiting, diarrhea, dyspepsia, flatulence; hypersensitivity reactions, exfoliative skin conditions including SJS; flu, headache, cutaneous vasculitis, acute pancreatitis. Worsening renal function. Upper resp tract infection, nasopharyngitis, constipation, headache.
