Indications/Uses

Antidote to folic acid antagonists. Palliative treatment of advanced CRC in combination w/ 5-fluorouracil.

Dosage/Direction for Use

Antidote for folic acid antagonists 6-12 mg IM every 6 hrly for 4 doses. Rescue after high-dose MTX therapy 10 mg/m²orally or parenterally followed by 10 mg/m²orally every 6 hr until serum MTX conc is <10^-8 M. If at 24-hr following MTX administration the serum creatinine is increased by 50% or greater than the pre-MTX serum creatinine Immediately increase to 100 mg/m²every 3 hr until serum MTX level is <5 x 10^-8 M. Advanced CRC 200 mg/m² by slow IV inj over a minimum of 3 min followed by 370 mg/m²of 5-fluorouracil or 20 mg/m²IV inj followed by 425 mg/m²of 5-fluorouracil. Treatment is repeated daily for 5 days. 5-day treatment course may be repeated at 4 wk intervals for 2 courses & then repeated at 4-5 wk intervals.

Overdosage

Contraindications

Neonates & infants. Vit B₁₂ deficiency anemia.

Special Precautions

Monitor serum MTX. Enhanced toxicity w/ 5-fluouracil. Debilitated CRC patients. Perform electrolytes & LFTs prior to each treatment for the 1st 3 cycles, then prior to every other cycle. Discontinue treatment if there is clear evidence of tumour progression. Pregnancy & lactation. Childn.
