Baclofen
Indications/Uses
Symptoms of spasticity from multiple sclerosis, particularly for relief of spasm & concomitant pain clonus & muscular rigidity. Dystonia; spasticity associated w/ cerebral palsy; trigeminal neuralgia.
Dosage/Direction for Use
Initially 5 mg tid for 3 days, increased to 10 mg tid for 3 days, then in similar increment & interval until 20 mg tid is reached or desired therapeutic effect is obtained. Childn Initially 2.5 mg given qid, increased about 3 days until desired therapeutic effect is obtained. Maintenance dose: 6-10 yr 30-60 mg daily, 2-6 yr 20-30 mg daily, 12 mth-2 yr 10-20 mg daily.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Epilepsy, rheumatic muscle spasm, preexisting bladder sphincter hypertonia.
Special Precautions
Patients w/ history of peptic ulcer; severe psychiatric disorders or epilepsy or convulsive disorders; resp impairment; DM; cerebrovascular disease; who use their spasticity to maintain posture or increase function. Avoid use in patients w/ active PUD. Monitor liver function in patients w/ liver disease. May exacerbate urine retention in patients w/ hypertonic bladder sphincters. Avoid abrupt w/drawal; gradually reduce dose over at least 1-2 wk. May affect ability to drive or operate machine. Renal impairment. Pregnancy. Not recommended during lactation. Elderly.
Adverse Reactions
Drowsiness, nausea, dizziness, lassitude, lightheadedness, confusion, fatigue, muscular pain & weakness, & hypotension.
Drug Interactions
CNS effects may be exacerbated w/ alcohol & other CNS depressants. Severe aggravation of hyperkinetic symptoms may occur w/ lithium. May increase weakness w/ TCA. May increase hypotensive effect of antihypertensives. Excretion may be reduced leading to toxicity w/ ibuprofen & other drugs producing renal insufficiency.
