Indications/Uses
Neuropathic pain associated w/ diabetic peripheral neuropathy. Postherpetic neuralgia & fibromyalgia. Adjunctive therapy for adult patients w/ partial onset seizures.
Dosage/Direction for Use
Max: 150-600 mg/day. Neuropathic pain associated w/ diabetic peripheral neuropathy CrCl at least 60 mL/min Initially 50 mg tid. May be increased to 300 mg/day w/in 1 wk. Max: 100 mg tid. Postherpetic neuralgia CrCl at least 60 mL/min 75-150 mg bid or 50-100 mg tid. Initially 75 mg bid or 50 mg tid, may be increased to 300 mg/day w/in 1 wk. Patient who does not experience sufficient pain relief following 2-4 wk w/ 300 mg/day 300 mg bid or 200 mg tid. Adjunctive therapy for adult patient w/ partial onset seizure 150-600 mg/day in 2 or 3 divided doses. Management of fibromyalgia 300-450 mg/day. Initially 75 mg bid, may be increased to 150 mg bid w/in 1 wk & may further increase to 225 mg bid. Doses >450 mg/day is not recommended. Neuropathic pain associated w/ spinal cord injury 150-600 mg/day. Initially 75 mg bid, may be increased to 150 mg bid w/in 1 wk. Patient who does not experience sufficient pain relief after 2-3 wk w/ 150 mg bid 300 mg bid. Renal impairment CrCl ≥60 mL/min 150, 300, 450 or 600 mg/day bid or tid, 30-60 mL/min 75, 150, 225 or 300 mg/day bid or tid, 15-30 mL/min 25-50, 75, 100-150 or 150 mg as single daily dose or bid, <15 mL/min 25, 25-50, 50-75 or 75 mg as single daily dose.
Administration
May be taken with or without food.
Special Precautions
Discontinue immediately if swelling of the face, mouth (tongue, lips & gums) & neck (throat & larynx); if myopathy is diagnosed or suspected or if markedly elevated creatinine kinase levels occurs. Diabetic patients. W/drawal of concomitant antiepileptic drugs. Suicidal ideation & behavior; peripheral edema; encephalopathy. Wt gain; blurred vision; decreased platelet count; PR interval prolongation, renal failure may occur. Patients w/ CHF NYHA class III or IV; history or potential of substance abuse. Concomitant use w/ drugs potential to produce constipation eg, opioid analgesics. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. May cause dizziness & somnolence & impair ability to drive or operate machinery. Pregnancy & lactation. Childn.
Adverse Reactions
Dizziness, trouble concentrating, blurry vision, swelling of hands & feet, wt gain, dry mouth, sleepiness.
