Per 4 g/500 mg powd for inj Piperacillin 4 g, tazobactam 500 mg
Indications/Uses
Intra-abdominal, skin & skin structures & female pelvic infections, community-acquired & nosocomial pneumonia.
Dosage/Direction for Use
IV Infuse over 30 min. Adult Usual total daily dose: 3.375 g every 6 hr totaling 13.5 g (12 g piperacillin/1.5 g tazobactam). Nosocomial pneumonia Initially 16 g piperacillin/2 g tazobactam. Childn >40 kg Adult dose, ≥9 mth weighing up to 40 kg 100 mg piperacillin/12.5 mg tazobactam per kg every 8 hr, 2-9 mth 80 mg piperacillin/10 mg tazobactam per kg every 8 hr.
Contraindications
Special Precautions
Patients w/ allergies to medicines, food or other substances; cystic fibrosis, bowel inflammation, bleeding or kidney problems, CHF, history of severe diarrhea or bowel problems w/ antibiotics. Patients on salt-restricted diet or low blood K levels. Patients taking any prescription or non-prescription medicine, herbal prep or dietary supplement. Pregnancy & lactation.
Adverse Reactions
Agitation, constipation, diarrhea, dizziness, headache, indigestion, nausea, pain, swelling or redness at inj site, sleeplessness, vomiting.
Drug Interactions
Concomitant use w/ aminoglycosides, OCs, anticoagulants, heparin, MTX, non-depolarizing muscle relaxants, probenecid, tetracyclines.
