Permethrin.
Description
Molecular Formula: C21H20Cl2O3.
Relative Molecular Mass: 391.3.
Shampoo: Clear yellow liquid.
Each mL contains 10 mg of Permethrin.
Lotion: White to off-white lotion, free of lumps, grit or foreign particles.
Each mL contains 50 mg of Permethrin.
Action
Shampoo: Pediculicide.
Lotion: Scabicide.
Pharmacology:
Pharmacodynamics: The potent insecticidal action of permethrin arises from repeated depolarization of the neurons and leads to a reduction in neuronal excitability causing sensory hyperexcitability, incoordination, prostration, paralysis and eventual death.
Pharmacokinetics: Permethrin is not rapidly absorbed transdermally and that the extent of systemic exposure is very low. Plasma concentrations of permethrin are undetectable. The small amount of permethrin that is absorbed is rapidly metabolized and excreted. Distribution of the drug into major body tissues is not of great concern.
Indications/Uses
Shampoo: For the treatment of head lice and their eggs (kuto at lisa) and prevention for those at risk.
Lotion: For the treatment of scabies (galis-aso) and crab lice infestation.
Dosage/Direction for Use
Shampoo: Directions for Use: Wet hair.
Shake bottle. Apply and lather enough Permethrin (Kwell) Shampoo to cover entire hair and scalp including areas behind ears and nape of the neck.
Leave for 10 minutes before rinsing the hair thoroughly with water. Towel dry.
Check any other family members and treat them if necessary to prevent re-infection.
Permethrin (Kwell) Shampoo is suitable for use by adults and children over 6 months.
In most cases, a single application is sufficient to eliminate head lice and provides continued protection for up to two weeks in 97-99% of cases.
Lotion: Directions for Use: Permethrin (Kwell) Lotion should be applied to clean, dry, cool skin. If warm bath was taken prior to treatment, allow skin to cool before applying Permethrin (Kwell) Lotion.
For Scabies: In adults and children over 2 years old: Permethrin (Kwell) Lotion should be applied to the whole body excluding the head and paying particular attention to the areas between the fingers and the toes and under fingernails and toenails, wrists, armpits, buttocks and genital area.
In children between 2 months and 2 years: Permethrin (Kwell) Lotion should be applied over the whole body as directed for the adults and children over 2 years, but should include the face, neck, scalp and ears. Care must be taken to avoid the vicinity of the mouth where it could be licked off and areas close to the eyes.
For Crab Lice: For adults and children over 2 months: Sufficient lotion should be applied to cover the pubic region, peri-anal area, inner thighs down to the knees and any hair on the trunk which extends to the pubic area. Any facial hair except the eyelashes and eyebrows should also be treated if found to be infested with lice or their eggs. It is recommended that any lice or their eggs found in eyelashes should be removed with a pair of tweezers.
A second treatment should be administered if live lice are found on any previously treated area 7 days after treatment.
During the treatment period, Permethrin (Kwell) Lotion should be re-applied to the hands if they are washed with soap and water.
The whole body should be washed thoroughly 8-12 hours after treatment.
Approximately 90% of individuals are cured with a single application of Permethrin (Kwell) Lotion.
Contraindications
Not to be used by individuals with known hypersensitivity to the product, its components, other pyrethroids, or pyrethrins.
Warnings
If accidentally swallowed by a child, consult a doctor immediately. Gastric lavage should be considered within two hours of ingestion and management should relate to treatment of alcoholic intoxication. For pregnant or lactating mothers, consult the doctor before use.
Shampoo: If eye contact occurs, wash out promptly & thoroughly with water. If severe or prolonged signs and symptoms of scalp irritation, skin discomfort or other undesirable effects occur in association with the use of Permethrin (Kwell) Shampoo, it should be brought to the attention of a doctor or pharmacist.
Lotion: If eye contact occurs, wash out promptly & thoroughly with water. The use of corticosteroids should be withheld prior to or during the treatment of Permethrin (Kwell) Lotion.
Storage
Store at temperatures not exceeding 30°C.
Shake well before use.
Protect from light.
