Clarithromycin
Indications/Uses
Lower resp tract infections (eg, acute & chronic bronchitis, & pneumonia) & URTI (eg, sinusitis & pharyngitis). Initial therapy in community acquired resp infections. Skin & soft tissue infections of mild to moderate severity eg, folliculitis, cellulitis, erysipelas. Eradication of H. pylori in patients w/ duodenal ulcers.
Dosage/Direction for Use
Resp tract/skin & soft tissue infection Adult 250 mg bid, may be increased to 500 mg bid in severe infections. Duration of treatment: 6-14 days. Childn >12 yr Same w/ adult dose. Eradication of H. pylori in patient w/ duodenal ulcers Adult Duration of treatment: 6-14 days. Triple therapy: Clarithromycin 500 mg bid & lansoprazole 30 mg bid w/ amoxicillin 1,000 mg bid; or clarithromycin 500 mg bid & lansoprazole 30 mg bid w/ metronidazole 400 mg bid; or clarithromycin 500 mg bid & omeprazole 40 mg daily w/ amoxicillin 1,000 mg bid or metronidazole 400 mg bid; or clarithromycin 500 mg bid & omeprazole 20 mg daily w/ amoxicillin 1,000 mg bid. Elderly Same w/ adult dose. Patient w/ renal impairment CrCl <30 mL/min Reduce dose by ½ ie, 250 mg once daily or 250 mg bid in more severe infections. Do not continue treatment beyond 14 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to clarithromycin or macrolides. Patients w/ history of QT prolongation (congenital or documented acquired QT prolongation) or ventricular cardiac arrhythmia, including torsades de pointes. Concomitant use w/ ergot alkaloids (eg, ergotamine or dihydroergotamine); oral midazolam; astemizole, cisapride, pimozide & terfenadine; ticagrelor or ranolazine.
Special Precautions
May select for drug-resistant organisms in the treatment of H. pylori infection. Patients w/ severe renal insufficiency; hepatic impairment; moderate to severe renal impairment. Pregnancy (particularly during 1st trimester) & lactation. Not recommended for childn <12 yr.
Adverse Reactions
Insomnia; dysgeusia, headache, taste perversion; diarrhoea, vomiting, dyspepsia, nausea, abdominal pain; abnormal LFT; rash, hyperhidrosis.
Drug Interactions
QT prolongation & cardiac arrhythmias including ventricular tachycardia, ventricular fibrillation & torsades de pointes w/ cisapride, pimozide, astemizole & terfenadine.
