Loperamide 2 mg, simethicone 125 mg
Indications/Uses
Control of acute diarrhea of any cause & its commonly associated symptoms.
Dosage/Direction for Use
Adult & childn ≥12 yr Initially 2 tab, followed by 1 tab after every loose stool. Max: 4 tab daily, limited to 2 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Not to be used as primary therapy in patients w/ acute dysentery; acute ulcerative colitis; bacterial enterocolitis caused by invasive organisms including Salmonella, Shigella & Campylobacter; pseudomembranous colitis associated w/ the use of broad-spectrum antibiotics. Not be used when inhibition of peristalsis is to be avoided due to possible risk of significant sequelae including ileus, megacolon & toxic megacolon. Promptly discontinue when constipation, abdominal distension or ileus develops. Childn <12 yr.
Special Precautions
Discontinue administration if clinical improvement is not observed w/in 48 hr. Stop therapy at the earliest signs of abdominal distension in AIDS patients. Abuse & misuse have been described in individuals w/ opioid addiction. Use w/ caution in patients w/ hepatic impairment as it may result in relative overdose leading to CNS toxicity.
Adverse Reactions
Nausea; dysgeusia. Loperamide: Headache, dizziness; abdominal discomfort/distension, upper abdominal pain.
Drug Interactions
Loperamide: Increased plasma levels w/ quinidine or ritonavir (P-gp inhibitors); itraconazole or ketoconazole (CYP3A4 & P-gp inhibitors); gemfibrozil (CYP2C8 inhibitor). Increased peak plasma levels & total plasma exposure w/ combination of itraconazole & gemfibrozil. Increased plasma conc of oral desmopressin. Drugs w/ similar pharmacological properties may potentiate loperamide's effect; drugs that accelerate GI transit may decrease its effect.
