Indications/Uses
Conditions where inhibition of gastric acid secretion may be beneficial including aspiration syndromes, dyspepsia, erosive esophagitis, GERD, benign gastric ulcer, duodenal ulcer, & hypersecretory conditions (eg, Zollinger-Ellison syndrome).
Dosage/Direction for Use
Dyspepsia 20 mg daily for 2-4 wk. GERD 20 mg once daily for 4 wk; some may require additional 4 wk of therapy. Refractory esophagitis 40 mg daily. Maintenance therapy: 20 mg once daily after healing of esophagitis. Childn Dose range: 0.7-1.4 mg/kg daily, up to max dose of 40 mg, have been given for 4-12 wk. PUD Single dose of 20 mg, or 40 mg in severe cases. Treatment is continued for 4 wk for duodenal ulcer & 8 wk for gastric ulcer. Where appropriate, dose of 20 mg once daily may be given for maintenance. NSAID-associated ulceration 20 mg once daily. Zollinger-Ellison syndrome Initially 60 mg once daily. Prophylaxis of acid aspiration during general anesth 40 mg in evening before surgery & 40 mg on the day of surgery, 2-6 hr before the procedure.
Administration
Should be taken on an empty stomach.
Contraindications
Hypersensitivity to omeprazole or substituted benzimidazoles.
Special Precautions
Consider possibility of malignancy in patients w/ gastric ulcers before giving omeprazole & other PPIs since these drugs may mask symptoms & delay diagnosis. Increased risk of GI infection. Caution in patients w/ hepatic impairment.
Adverse Reactions
Headache, diarrhea, skin rashes. Pruritus, dizziness, fatigue, constipation, nausea, vomiting, flatulence, abdominal pain, arthralgia, myalgia, urticaria, dry mouth.
Drug Interactions
May prolong elimination of diazepam, phenytoin, & warfarin. Reduced absorption of drugs (eg, ketoconazole, & possibly itraconazole) whose absorption is dependent on acid gastric pH. May alter metabolism of other drugs metabolized by CYP450, primarily CYP2C19.
