Cryptococcosis; systemic, mucosal, genital candidiasis; deep endemic mycoses; superficial fungal infections. Prophylaxis of transplant patients, patients w/ malignancy who are receiving cytotoxic chemotherapy, immunosuppressive & RT. Prophylaxis of HIV/AIDS in patients who are at risk of developing candida infections & serious fungal infections.

Adult Cryptococcal meningitis & other cryptococcal infections 400 mg on 1st day, then 200-400 mg once daily for 6-8 wk up to 10-12 wk. Oropharyngeal candidiasis 50-100 mg once daily for 7-14 days. Denture-associated atrophic oral candidiasis 50 mg once daily for 14 days. Esophageal candidiasis 50-100 mg once daily for 14-30 days, may be increased to 400 mg once daily if necessary. Mucosal candidiasis 50-100 mg once daily for 14-30 days. Systemic candida infections 400 mg on 1st day, then 200 mg once daily, may be increased to 400 mg once daily. Treatment should be continued for min of 4 wk & at least 2 wk after symptoms have resolved. Vag candidiasis & candidal balanitis 150 mg single dose. Recurrent vulvovag candidiasis 150 mg on 1st day, then 150 mg on the 4th day, maintenance: 100-150 mg once a wk for 6 mth. Dermatologic infections including tinea pedis, corporis & cruris; candida infections 150 mg once a wk or 50 mg once daily for 2-6 wk. Tinea versicolor & capitis 50 mg once daily for 2-4 wk. Tinea unguium 150 mg once a wk (fingernails: 3-6 mth & toenails: 6-12 mth). Deep endemic mycoses 200-800 mg once daily (coccidioidomycosis: 11-24 mth; paracoccidioidomycosis: 2-17 mth; sporotrichosis: 1-16 mth & histoplasmosis: 3-17 mth). Suppression of relapse of cryptococcal meningitis & cryptococcal infections 100-200 mg once daily. Prevention of oropharyngeal candidiasis relapse in patient w/ AIDS 100-200 mg once a wk for 4-9 mth. Prevention of fungal infections in patient w/ HIV 100-200 mg once daily. Prevention of fungal infections in transplant patient, patient undergoing chemotherapy, immunotherapy or RT & other patient who are at risk of systemic fungal infections 400 mg once daily. Childn 3-12 mg/kg once daily, max: 600 mg once daily. Oropharyngeal candidiasis 6 mg/kg once daily, then 3 mg/kg once daily for at least 2 wk to decrease likelihood of relapse. Esophageal candidiasis 6 mg/kg on 1st day, then 3 mg/kg once daily, may be increased to 12 mg/kg/day. Systemic candida infections 6-12 mg/kg daily. Cryptococcal meningitis & infections on other sites 12 mg/kg on 1st day, then 6 mg/kg once daily, may be increased to 12 mg/kg/day for 10-12 wk. Suppression of cryptococcal meningitis in patient w/ AIDS 6 mg/kg once daily for 6 mth or longer. Infant <2 wk Once every 3 days; 2-4 wk Once every other day.

May be taken with or without food.

Hypersensitivity to fluconazole & other azole antifungals.

Discontinue if abnormal LFTs; rash; bullous lesions or erythema occur. Overgrowth of nonsusceptible strains of candida. Anaphylaxis. QT prolongation & torsades de pointes. Pregnancy & lactation. Elderly & childn.

Abdominal pain, diarrhea, flatulence, nausea, vomiting & taste disturbance. Headache, dizziness, eosinophilia, anemia, leukopenia, thrombocytopenia, hyperlipidemia & raised liver enzyme conc.

Reduced AUC & C_max w/ cimetidine. Significant increase in AUC & C_max w/ hydrochlorothiazide. Decreased AUC & shorter t_½ w/ rifampicin. Elevation in serum levels of aztemizole & rifabutin. Increased AUC metabolite of terfenadine. Increase in the level of OC components. Significant increase in AUC, C_max & C_max of cyclosporine. Increased AUC values of phenytoin. Significant increase in AUC, t_½ & C_max & decreased in clearance of theophylline. Significant increase in AUC & C_max of sulfonylureas (tolbutamide, glibenclamide, glipizide). Increased PT response (area under prothrombin time-time curve) w/ warfarin. Significant increase in zidovudine. Significant increase in the QT interval w/ cisapride.

Antifungals

J02AC01 - fluconazole ; Belongs to the class of triazole and tetrazole derivatives. Used in the systemic treatment of mycotic infections.
