Indications/Uses
Replacement therapy in the treatment of hypothyroidism, to suppress TSH production in the treatment of thyroid carcinoma & as a diagnostic agent for differential diagnosis of hypothyroidism.
Dosage/Direction for Use
Hypothyroidism Adult Initially 50-100 mcg daily, may be increased by 25-50 mcg at 4-wk intervals until thyroid deficiency is corrected. Maintenance dose: 100-200 mcg daily. Patient w/ CV disorder, long-standing severe hypothyroidism or elderly Initially 12.5-50 mcg daily, increased by increments of 12.5-25 mcg at 4-wk intervals. Congenital hypothyroidism Childn 10-15 mcg/kg daily, adjusted as necessary. Alternatively, an initial dose of 5-10 mcg/kg daily in childn up to 1 mth of age (or 5 mcg/kg in childn >1 mth), adjusted in steps of 25 mcg every 2-4 wk until mild toxic symptoms appear, at which time dosage is slightly reduced. Juvenile myxedema Childn >1 yr Initially 2.5-5 mcg/kg daily.
Administration
Should be taken on an empty stomach: Take 30 min-1 hr before meals.
Contraindications
Hypersensitivity. Thyrotoxicosis.
Special Precautions
Contraindicated in untreated hyperthyroidism. Not to be given to patients w/ adrenal insufficiency w/o adequate corticosteroid cover. Caution in patients w/ CV disorders including angina, heart failure, MI & HTN; DM or diabetes insipidus; impaired adrenocortical function. Gradually introduce treatment in elderly & those w/ long-standing hypothyroidism. Test if thyroid functions are subject to alteration by a number of nonthyroidal clinical conditions & by a wide variety of drugs. Pregnancy & lactation.
Adverse Reactions
Anginal pain, cardiac arrhythmias, palpitation & cramps in skeletal muscle; tachycardia, diarrhea, vomiting, tremors, restlessness, excitability, insomnia, headache, flushing, sweating, excessive wt loss & muscular weakness.
Drug Interactions
Increased effect of anticoagulants. May increase levels of phenytoin. Anticonvulsants eg, carbamazepine & phenytoin enhance the metabolism of thyroid hormones & may displace them from plasma proteins. Dose adjustment of cardiac glycoside maybe necessary. Enhanced effects of sympathomimetic agents. Accelerated response to TCAs. Reduced GI absorption w/ cholestyramine. Co-administration of OCs may result in increased dosage requirement of thyroid therapy. A number of drugs may affect thyroid function tests.
