Indications/Uses
Infections caused by bacteria known or very likely to be susceptible to ertapenem & when parenteral therapy is required (eg, intra-abdominal & acute gynaecological infections, community acquired pneumonia, diabetic foot infections of the skin & soft tissue) in adults & paed patients 3 mth-17 yr. Prophylaxis of surgical site infection following elective colorectal surgery in adults.
Dosage/Direction for Use
Treatment Adult & and adolescent 13-17 yr 1 g once daily by IV infusion over 30 min for 3-14 days. Infant & childn 3 mth-12 yr 15 mg/kg bid by IV infusion over 30 min for 3-14 days. Max: Not to exceed 1 g daily. Prevention of surgical site infection following elective colorectal surgery Adult 1 g as single IV dose completed w/in 1 hr prior to the surgical incision.
Contraindications
Hypersensitivity to ertapenem or other carbapenem antibacterial agent. Severe hypersensitivity (eg, anaphylactic & severe skin reaction) to any other type of β-lactam antibacterial agent eg, penicillins or cephalosporins.
Special Precautions
Discontinue use if an allergic reaction occurs. Prolonged use may result in overgrowth of non-susceptible organisms. Antibiotic-associated & pseudomembranous colitis. Seizures occurred most commonly in elderly patients & those w/ pre-existing CNS disorders (eg, brain lesions or history of seizures) &/or compromised renal function. Patients could be exposed to sub-optimal conc & consequently risk of potential treatment failure in very few cases of surgical interventions >4 hr. Patients on a controlled Na diet. Limited experience in severe infections. Efficacy has not been established in community-acquired pneumonia due to penicillin-resistant Strep pneumoniae; diabetic foot infections w/ concurrent osteomyelitis. Not to be given w/ medicinal products that inhibit peristalsis. Not recommended in concomitant use w/ valproic acid/Na valproate. Not to be used in patients w/ severe renal impairment; on haemodialysis. No adequate & well-controlled studies regarding effect on fertility in men & women. Pregnancy. Do not breast-feed during treatment. Safety & efficacy have not yet been established in childn <3 mth. No data in childn & adolescents w/ renal impairment.
Adverse Reactions
Diarrhoea. Adults: Infused vein complication & nausea. Paed population 3 mth-17 yr: Infusion site pain.
Drug Interactions
Decreased levels of valproic acid that may fall below therapeutic range.
