Per 4 g/500 mg powd for inj Piperacillin 4 g, tazobactam 500 mg
Indications/Uses
Intra-abdominal infections, skin & skin structures infections, female pelvic infections, community-acquired pneumonia & nosocomial pneumonia.
Dosage/Direction for Use
Administer by IV infusion over 30 min. Adult General infection 3.375 g every 6 hr totaling 13.5 g (12 g piperacillin/1.5 g tazobactam). Initial presumptive treatment of nosocomial pneumonia 4.5 g every 6 hr totaling 18 g (16 g piperacillin/2 g tazobactam) + aminoglycoside. Ped patient ≥9 mth Appendicitis &/or peritonitis 100 mg piperacillin/12.5 mg tazobactam/kg every 8 hr. Ped patient 2-9 mth Appendicitis &/or peritonitis 80 mg piperacillin/10 mg tazobactam/kg every 8 hr. Adult w/ renal impairment (CrCl ≤40 mL/min) & dialysis patient Reduce dose according to the degree of renal function impairment.
Contraindications
Special Precautions
Before initiating therapy, careful inquiry should be made concerning previous hypersensitivity reactions to penicillins, other β-lactam agents (eg, cephalosporin, monobactam or carbapenem) & other allergens. Reports of serious & occasionally fatal hypersensitivity; more likely to occur in persons w/ history of sensitivity to multiple allergens.
Adverse Reactions
Agitation, constipation, diarrhea, dizziness, headache, indigestion, nausea, pain, swelling or redness at the inj site, sleeplessness, & vomiting.
Drug Interactions
Potential interaction w/ aminoglycosides, OCs, anticoagulants, heparin, MTX, non-depolarizing muscle relaxants, probenecid, & tetracyclines.
