Indications/Uses
Treatment of rocky mountain spotted fever, typhus fever & typhus group, Q fever, rickettsial pox & tick fevers caused by Rickettsiae; resp tract infections; lymphogranuloma venereum; psittacosis; trachoma; inclusion conjunctivitis; uncomplicated urethral, endocervical or rectal infections in adult; nongonococcal urethritis; relapsing fever. Treatment of infections caused by gm -ve microorganisms: Chancroid, plague, tularemia, cholera, Campylobacter fetus infections, brucellosis in conjunction w/ streptomycin, bartonellosis, granuloma inguinale.  Treatment of infections caused by E. coli, Enterobacter aerogenes, Shigella spp, Acinetobacter spp, resp tract infections caused by H. influenzae; resp tract infections & UTI caused by Klebsiella spp; upper resp tract infections caused by S. pneumoniae; anthrax. Alternative treatment of uncomplicated gonorrhea, syphilis, yaws, listeriosis, Vincent's infection, actinomycosis, infections caused by Clostridium spp. Adjunct to amebicides; severe acne.
Dosage/Direction for Use
Adult 200 mg on the 1st day of treatment (administered 100 mg every 12 hr) followed by maintenance dose of 100 mg/day. Chronic infections of urinary tract 100 mg every 12 hr. Childn <45 kg w/ anthrax, rocky mountain spotted fever 2.2 mg/kg every 12 hr. ≥45 kg Adult dose. Childn w/ less severe disease >8 yr & <45 kg 4.4 mg/kg in 2 divided doses on the 1st day of treatment followed by maintenance dose of 2.2 mg/kg. Uncomplicated gonococcal infections in adults (except anorectal infections in men) 100 mg bid for 7 days. As an alternative single dose, administer 300 mg stat followed in 1 hr by a 2nd 300 mg dose. Uncomplicated urethral, endocervical or rectal infection in adults caused by C. trachomatis, nongonococcal urethritis (NGU) caused by C. trachomatis or U. urealyticum 100 mg bid for 7 days. Syphilis (early) 100 mg bid for 2 wks. Syphilis >1 yr duration 100 mg bid for 4 wks. Acute epididymo-orchitis caused by N. gonorrhoeae & C. gonorrhoeae 100 mg bid for 10 days. Prophylaxis of malaria Adult 100 mg daily, childn >8yr 2 mg/kg once daily up to the adult dose. Prophylaxis should begin 1-2 days before travel to the malarious area. Should be continued daily during travel in the malarious area & 4 wks after the traveller leaves the malarious area. Inhalational anthrax (post-exposure) Adult 100 mg bid for 60 days, childn ≥45 kg Adult dose, <45 kg 2.2 mg/kg bid for 60 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to tetracyclines.
Special Precautions
May result to overgrowth. Discontinue if superinfection, phototoxicity occurs. May increase incidence of vag candidiasis. Diarrhea. Pregnancy & lactation. Child <8 yr.
Adverse Reactions
Anorexia, nausea, vomiting, diarrhea, glossitis, dysphagia, enterocolitis, inflammatory lesions (w/ monilial overgrowth) in the anogenital region, pancreatitis; hepatotoxicity (rarely); permanent tooth discoloration & enamel hypoplasia when used during tooth development. TEN, SJS, erythema multiforme, skin hyperpigmentation, maculopapular & erythematous rashes; exfoliative dermatitis; photosensitivity. Rise in BUN. Hypersensitivity including urticaria, angioneurotic edema, anaphylaxis, anaphylactoid purpura, serum sickness, pericarditis, exacerbation of SLE, DRESS, Jarisch-Herxheimer reaction. Hemolytic anemia, thrombocytopenia, neutropenia & eosinophilia. Bulging fontanels in infants & intracranial HTN in adults.
Drug Interactions
Decrease plasma prothrombin w/ anticoagulants. May interfere w/ the bactericidal action of penicillin. Impaired absorption w/ antacids containing Al, Ca or Mg & Fe-containing prep; bismuth subsalicylate. Decrease t½ w/ barbiturates, carbamazepine & phenytoin. Fatal renal toxicity w/ methoxyflurane. May render OC less effective. False elevations of urinary catecholamine levels w/ fluorescence test.
