Indications/Uses
FC tab: Infections in adults & childn ≥3 mth: Acute streptococcal tonsillitis & pharyngitis; acute bacterial sinusitis; acute otitis media; acute exacerbations of chronic bronchitis; cystitis; pyelonephritis; uncomplicated skin & soft tissue infections; early Lyme disease. Powd for susp: Bone & joint infections, bronchitis (& other lower resp tract infections), gonorrhea, meningitis, otitis media, peritonitis, pharyngitis, sinusitis, skin infections (including soft tissue infections) & UTI. Surgical infection prophylaxis.
Dosage/Direction for Use
FC tab Adult & adolescent ≥13 yr Most infection 250 mg every 12 hr. Severe infection 500 mg every 12 hr. Acute sinusitis, acute tonsillopharyngitis, acute bacterial exacerbation of chronic bronchitis, uncomplicated skin & skin structure infection 250 or 500 mg every 12 hr for 10 days. Acute otitis media 500 mg every 12 hr for 10 days. Secondary bacterial infection of acute bronchitis 250 or 500 mg every 12 hr for 5-10 days. Uncomplicated UTI including pyelonephritis 250 or 500 mg every 12 hr for 7-10 days. Uncomplicated gonorrhea Single dose of 1,000 mg. Early Lyme disease 500 mg every 12 hr for 20 days. Powd for susp Usual treatment duration: 7 days (range: 5-10 days). Adult Most infection, mild to moderate lower resp tract infection (eg, bronchitis), pyelonephritis 250 mg every 12 hr. UTI 125 mg every 12 hr. More severe lower resp tract infection, or if pneumonia is suspected 500 mg every 12 hr. Uncomplicated gonorrhea Single dose of 1 g. Ped 3 mth-12 yr Pharyngitis/tonsillitis 20 mg/kg bid divided in equal doses for 10 days. Max: 500 mg/day. Acute bacterial otitis media, acute bacterial maxillary sinusitis (mild to moderate), impetigo 30 mg/kg bid divided in equal doses for 10 days. Max: 1,000 mg/day.
Administration
Should be taken with food: Take after meals.
Contraindications
Hypersensitivity to cefuroxime or other cephalosporins. FC tab: History of severe hypersensitivity (eg, anaphylactic reaction) to any other type of β-lactam antibacterial agent (penicillins, monobactam, carbapenems).
Special Precautions
Pregnancy & lactation. FC tab: Immediately discontinue treatment & initiate adequate emergency measures in case of severe hypersensitivity reactions. Patients who have experienced allergic reaction to penicillins or other β-lactam antibiotics; patients w/ history of non-severe hypersensitivity to other β-lactam agents. Jarisch-Herxheimer reaction has been seen following cefuroxime axetil treatment of Lyme disease. May result in overgrowth of Candida. Prolonged use may result in overgrowth of other non-susceptible microorganisms (eg, Enterococci & Clostridium difficile). Reports of pseudomembranous colitis. Powd for susp: Patients w/ history of allergy. Monitor renal hematological status especially during prolonged & high-dose therapy. Patients w/ renal impairment.
Adverse Reactions
FC tab: Candida overgrowth; eosinophilia; headache, dizziness; diarrhea, nausea, abdominal pain; transient increases of hepatic enzyme levels. Powd for susp: GI disturbances including diarrhea, nausea, & vomiting.
Drug Interactions
Drugs which reduce gastric acidity may result in lower bioavailability of cefuroxime axetil & tend to cancel the effect of enhanced absorption after food. Reduced renal clearance & significantly increased peak conc, AUC & elimination t1/2 w/ probenecid. FC tab: May affect gut flora, leading to lower estrogen reabsorption & reduced efficacy of combined OCs. Concomitant use w/ oral anticoagulants may give rise to increased INR.
