Cefixime trihydrate
Indications/Uses
URTI eg, bacterial pharyngitis, tonsilitis, otitis media, sinusitis; lower resp tract infections eg, acute bronchitis; UTI eg, acute uncomplicated cystitis & urethritis; uncomplicated gonorrhea.
Dosage/Direction for Use
Adult 400 mg once daily or 200 mg every 12 hr. Uncomplicated cervical/urethral gonococcal infection 400 mg as single dose. Childn 8 mg/kg/day as single dose or in 2 divided doses as 4 mg/kg every 12 hr. Typhoid 15-20 mg/kg/day as single dose or divided into 2 equal doses every 12 hr for 7-14 days. Infection due to Strep pyogenes Administer therapeutic dosage for at least 10 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to cephalosporins. Renal impairment w/ CrCl <60 mL/min.
Special Precautions
Patients who have shown hypersensitivity to other medicines/drugs; penicillin-sensitive patients. Discontinue if allergic effect occurs. Patients w/ markedly impaired renal function. Prolonged use may result in overgrowth of non-susceptible organisms. Antibiotic-associated pseudomembranous colitis; discontinue if diarrhea occurs. Encephalopathy. Discontinue when severe cutaneous adverse reactions eg, TEN, SJS & DRESS occur. Drug-induced hemolytic anaemia, including severe cases w/ fatal outcome. Discontinue when acute renal failure including tubulointerstitial nephritis occurs. Renal impairment. Not to be used in pregnancy or lactation. Premature or newborn infant.
Adverse Reactions
Diarrhea & stool changes; nausea, abdominal pain, dyspepsia, vomiting & flatulence; pseudomembranous colitis. Headache & dizziness. Hypersensitivity reactions in the form of rash, pruritus, urticaria, drug fever & arthralgia. Thrombocytopenia, leukopenia & eosinophilia; changes in liver & renal function tests. Genital pruritus & vaginitis.
Drug Interactions
May enhance effects of coumarin-type anticoagulants eg, warfarin K, & prolonged prothrombin time w/ or w/o bleeding may occur. False +ve reaction for glucose in urine may occur w/ Benedict's or Fehling's soln or w/ Cu sulphate test tab. False +ve direct Coombs test.
