Cefixime
Indications/Uses
URTI eg, otitis media; & other URTI where the causative organism is known or suspected to be resistant to other commonly used antibiotics, or where treatment failure may carry significant risk. Lower resp tract infection eg, bronchitis. UTI eg, cystitis, cystourethritis, uncomplicated pyelonephritis. Infections caused by commonly occurring pathogens including Strep pneumoniae, Strep pyogenes, E. coli, Proteus mirabilis, Klebsiella spp, H. influenzae (β-lactamase +ve & -ve), Branhamella catarrhalis (β-lactamase +ve & -ve) & Enterobacter spp.
Dosage/Direction for Use
Adult, elderly, & adolescent ≥12 yr 200-400 mg daily as a single dose according to severity of infection (400 mg may also be taken as 2 divided doses). Duration of treatment: 7 days. May be continued for up to 14 days if required. Childn 6 mth-11 yr 8 mg/kg/day as a single dose or in 2 divided doses. Adult w/ CrCl <20 mL/min; patient who is maintained on chronic ambulatory peritoneal dialysis or hemodialysis 200 mg once daily should not be exceeded.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to cefixime or other cephalosporin antibiotics. Previous, immediate &/or severe hypersensitivity reaction to penicillin or any β-lactam antibiotic.
Special Precautions
Discontinue use if severe hypersensitivity reactions or anaphylactic reactions occur; in patients who develop severe diarrhea during or after use of cefixime. Caution in patients who have shown hypersensitivity to other drugs. Cross-allergenicity may occur between cephalosporins, penicillins & β-lactam antibiotics. Prolonged use may result in overgrowth of non-susceptible organisms. Consider pseudomembranous colitis in patients who develop diarrhea in association w/ use of broad-spectrum antibiotics. Caution in patients w/ CrCl <20 mL/min. Not to be used in pregnant mothers unless considered essential. Not to be prescribed to breastfeeding mothers until further clinical experience is available. Not recommended in ped & adolescent w/ renal insufficiency.
Drug Interactions
False +ve reaction for glucose in urine may occur w/ Benedict's or Fehling's soln or Cu sulphate test tab. False +ve direct Coombs' test. Increases in prothrombin time w/ anticoagulation therapy.
