Indications/Uses
Management of renal tubular acidosis w/ Ca stones, hypocitraturic Ca oxalate nephrolithiasis of any etiology, & uric acid lithiasis w/ or w/o Ca stones.
Dosage/Direction for Use
Severe hypocitraturia (urinary citrate <150 mg/day) 20 mEq tid or 15 mEq qid (60 mEq/day). Mild to moderate hypocitraturia (>150 mg/day) 10 mEq tid (30 mEq/day).
Administration
Should be taken with food: Take w/ meals or w/in 30 min after meals/bedtime snack. Swallow whole, do not crush/chew/suck tab.
Contraindications
Patients w/ hyperkalemia or who have conditions predisposing to hyperkalemia eg, chronic renal failure, uncontrolled DM, acute dehydration, strenuous physical exercise in unconditioned individuals, adrenal insufficiency, extensive tissue breakdown, or the administration of K-sparing agent (eg, triamterene, spironolactone or amiloride). Patients suffering from delayed gastric emptying, esophageal compression, intestinal obstruction or stricture or those taking anticholinergic medication. Not to be given to patients w/ PUD. Renal insufficiency (GFR <0.7 mL/kg/min).
Special Precautions
May produce hyperkalemia & cardiac arrest in patients w/ impaired mechanisms for excreting K. Avoid use in patients w/ chronic renal failure, or any other condition which impairs K excretion eg, severe myocardial damage or heart failure. Discontinue immediately & investigate the possibility of bowel perforation or obstruction if there is severe vomiting, abdominal pain or GI bleeding. Perform 24-hr urinary citrate &/or urinary pH measurements every 4 mth to determine adequacy of initial dosage & evaluate effectiveness of any dosage change. Should be given to pregnant & breastfeeding women only if clearly needed. 1,080 mg: Safety & effectiveness in childn have not been established.
Adverse Reactions
Abdominal discomfort, vomiting, diarrhea, loose bowel movements or nausea.
Drug Interactions
Concomitant administration w/ K-sparing diuretic (eg, triamterene, spironolactone or amiloride).
