Terbutaline sulfate
Indications/Uses
Bronchial asthma. Chronic bronchitis, emphysema & other lung diseases where bronchospasm is a complicating factor.
Dosage/Direction for Use
Individualized dosage. Adult 2.5 mg 3 times in 24 hr during the 1st 1-2 wk. May then be increased to 5 mg 3 times in 24 hr if necessary. Childn 0.075 mg/kg 3 times in 24 hr. Not more than 5 mg 3 times in 24 hr.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Caution in patients w/ thyrotoxicosis; underlying severe heart disease (eg, ischaemic heart disease, arrhythmia or severe heart failure); acute severe asthma. Assess symptoms eg, dyspnoea & chest pain as they may be of either resp or cardiac origin. Additional blood glucose controls are recommended initially in diabetic patients. Risk of serious hypokalemia. Patients w/ persistent asthma who require maintenance therapy w/ β2-agonists should also receive optimal anti-inflammatory therapy eg, inhaled corticosteroids, leukotriene receptor antagonists, & continued after introduction of Bricanyl even when symptoms decrease. Caution during the 1st trimester of pregnancy, & if used for maintenance, caution at the end of pregnancy because of potential tocolytic effect. Reports of transient hypoglycemia in newborn preterm infants after maternal β2-agonist treatment.
Adverse Reactions
Tremor, headache. Tachycardia, palpitations; tonic muscle cramps; hypokalaemia.
Drug Interactions
Partial or total inhibition of effect w/ β-receptor blocking agents (including eye-drops), especially those which are non-selective. Avoid use w/ halothane anaesth due to increased risk of cardiac arrhythmias; caution w/ other halogenated anaesth. Increased risk of hypokalemia w/ serum K depleting agents eg, diuretics, methyl xanthines & corticosteroids. Hypokalaemia also predisposes to digoxin toxicity.
