Biperiden HCl
Indications/Uses
Symptomatic treatment of parkinsonism including alleviation of extrapyramidal syndrome induced by drugs eg, phenothiazines.
Dosage/Direction for Use
Parkinson's disease Initially 2 mg tid or qid, increased according to response to max of 16 mg daily. Drug-induced extrapyramidal symptom 2 mg once daily to tid.
Administration
Should be taken with food.
Contraindications
Hypersensitivity. Narrow angle glaucoma; bowel obstruction; megacolon.
Special Precautions
Not to be given to patients w/ angle-closure glaucoma or w/ narrow angle between the iris & the cornea. Isolated instances of mental confusion, euphoria, agitation & disturbed behaviour. Risk of central anticholinergic syndrome. Patients w/ manifest glaucoma; prostatism, epilepsy or cardiac arrhythmia; acute MI & HTN. May raise IOP & precipitate acute attack. Caution in conditions characterized by tachycardia eg, thyrotoxicosis, heart failure, & in cardiac surgery. May cause confusion. Reduced bronchial secretion. Avoid abrupt w/drawal. Avoid consumption of alcohol during therapy. Drowsiness may occasionally occur which may affect ability to drive or operate machinery. Pregnancy & lactation. Safety & effectiveness have not been established in childn. Elderly.
Adverse Reactions
Dryness of mouth w/ difficulty in swallowing & talking, thirst, reduced bronchial secretions, mydriasis w/ cycloplegia & photophobia, flushing & dryness of skin, transient bradycardia followed by tachycardia w/ palpitations & arrhythmias, difficulty in micturition, reduction in tone & motility of the GIT leading to constipation.
Drug Interactions
Effects may be enhanced w/ other drugs w/ antimuscarinic properties eg, amantadine, some antihistamines, phenothiazine, antipsychotics, & TCAs; MAOIs. May antagonize GI effects of cisapride, domperidone, & metoclopramide. Concomitant use w/ parasympathomimetics may counteract each other's effects
