Carvedilol

Indications/Uses

Management of HTN & angina pectoris. Adjunct to standard therapy in symptomatic heart failure.

Dosage/Direction for Use

HTN Initially 12.5 mg once daily, increased to 25 mg once daily after 2 days. Alternatively, initial dose of 6.25 mg bid, increased to 12.5 mg bid after 1-2 wk. May be further increased to 50 mg once daily or in divided doses at intervals of at least 2 wk. Elderly 12.5 mg once daily. Angina pectoris Initially 12.5 mg bid, increased to 25 mg bid after 2 days. Heart failure Initially 3.125 mg bid. If tolerated, double the dose to 6.25 mg bid after 2 wk & then increase gradually at intervals of not <2 wk. Patient w/ mild to moderate heart failure >85 kg 50 mg bid, severe heart failure or <85 kg Max: 25 mg bid.

Overdosage

View Betacard overdosage for action to be taken in the event of an overdose.

Administration

Should be taken with food.

Contraindications

Hypersensitivity. Unstable/decompensated heart failure; clinically manifest liver dysfunction; 2nd- or 3rd-degree AV block (unless permanent pacemaker is in place); severe bradycardia (<50 bpm); sick sinus syndrome (including SA block); severe hypotension (systolic <85 mmHg); cardiogenic shock & history of bronchospasm or asthma; metabolic acidosis; severe peripheral arterial disease.

Special Precautions

Not to be given to patients w/ bronchospasm or asthma or those w/ history of obstructive airways disease. 1st-degree AV block. Pheochromocytoma w/o concomitant α-adrenoceptor blocking therapy. Abrupt w/drawal may exacerbate angina. Reduced peripheral circulation can produce coldness of extremities & may exacerbate peripheral vascular disease eg, Raynaud's syndrome. Pregnancy. Not recommended during lactation.

Adverse Reactions

Heart failure, heart block, & bronchospasm; bradycardia & hypotension.

Drug Interactions

Enhanced antihypertensive effects w/ ACE inhibitors, Ca-channel blocker, & clonidine; aldesleukin & general anaesth. Antagonized antihypertensive effects w/ NSAIDs. Precipitate bradycardia & heart block w/ other cardiac depressants eg, antiarrhythmics & rate-limiting Ca-channel blockers. May potentiate bradycardia due to digoxin. Reduced response to insulin & oral hypoglycaemics in diabetic patients. May develop elevated BP w/ adrenaline (epinephrine). Reduced absorption w/ Al salts & bile acid binding resins eg, colestyramine. Increased metabolism w/ barbiturates & rifampicin. Decreased metabolism w/ cimetidine, erythromycin, fluvoxamine, & hydralazine. Affected metabolism w/ drugs that alter hepatic blood flow.
