Azithromycin dihydrate
Indications/Uses
Infections from resp pathogens (eg, S. pyogenes, S. pneumoniae, M. catarrhalis, C. trachomatis, Legionella sp., Mycoplasma pneumoniae, S. aureus, & H. influenzae); C. pneumoniae & M. avium infection; uncomplicated chlamydial urethritis, cervicitis or pharyngitis; alternative drug or multi-drug resistant Salmonella typhi infection outside the CNS.
Dosage/Direction for Use
Adult Upper & lower resp tract & skin & soft tissue infection 500 mg once daily followed by 250 mg once daily for the next 4 days. Urogenital infections As a single dose.
Administration
May be taken with or without food: May be taken w/ meals to reduce GI discomfort.
Contraindications
Hypersensitivity to azithromycin dihydrate, erythromycin, any macrolide or ketolide antibiotic.
Special Precautions
Angioneurotic oedema & anaphylaxis (rarely fatal), dermatologic reactions including acute generalised exanthematous pustulosis, SJS, TEN (rarely fatal) & DRESS. Discontinue if allergic reaction occurs. C. difficile associated diarrhea. Prevention of acute rheumatic fever. Increased risk for ventricular arrhythmias including torsade de pointes which can lead to cardiac arrest especially in patients w/ ongoing proarrhythmic conditions (especially women & elderly) eg, patients w/ congenital or documented acquired QT prolongation; currently receiving treatment w/ other active substances known to prolong QT interval (eg, class IA & III antiarrhythmics, cisapride & terfenadine, antipsychotic agents, antidepressants & fluoroquinolones); w/ electrolyte disturbance particularly in cases of hypokalemia & hypomagnesemia; w/ clinically relevant bradycardia, cardiac arrhythmia or severe cardiac insufficiency. Myasthenia gravis. Should not be co-administered w/ ergot derivatives. Hepatic impairment. Severe renal impairment (GFR <10 mL/min). Pregnancy. Discontinue lactation during treatment. Prevention or treatment of mycobacterium avium complex in childn.
Adverse Reactions
GI disturbances. Transient elevations of liver enzyme values. Rashes, headache & dizziness. Transient alterations in neutrophil counts.
Drug Interactions
Reduced peak serum levels w/ antacids. Increased serum levels of the P-gp substrate w/ digoxin. Increase conc of phosphorylated zidovudine. Possibility of ergotism w/ ergotamine derivatives. Enhanced effects w/ astemizole & alfenatil. Post-marketing cases of rhabdomyolysis w/ atorvastatin. Increased QT interval prolongation, ventricular arrhythmias & torsades de pointes w/ cisapride. Potentiated anticoagulation of coumarin type oral anticoagulants. Elevated Cmax & AUC0-5 w/ cyclosporine. Decreased Cmax w/ fluconazole. Increased conc w/ nelfinavir.
