Azithromycin dihydrate

Indications/Uses

Resp-tract infections (including otitis media), skin & soft-tissue infections, uncomplicated genital infections. Prophylaxis & as component of regimens in treatment of Mycobacterium avium complex (MAC) infections.

Dosage/Direction for Use

Adult 500 mg as single dose for 3 days. Alternatively, may give initial dose of 500 mg followed by 250 mg daily for 4 days. Uncomplicated genital infections caused by Chlamydia trachomatis & chancroid 1 g as single dose. Uncomplicated gonorrhea 2 g as single dose. Granuloma inguinale Initially 1 g followed by 500 mg daily or 1 g once a wk for 3 wk until all lesions have completely healed. Prophylaxis of disseminated MAC infections 1.2 g once a wk. Treatment or secondary prophylaxis 500 mg once daily w/ other antimycobacterials. Mild or moderate typhoid 500 mg once daily for 7 days. Childn >45 kg Usual adult dose. Acute otitis media 30 mg/kg as single dose. Pharyngitis or tonsillitis Childn >2 yr12 mg/kg once daily for 5 days. Prophylaxis of disseminated MAC infections 20 mg/kg (max: 1.2 g) once wkly or 5 mg/kg (max: 250 mg) once daily. Treatment of disseminated MAC infections 10-12 mg/kg (max: 500 mg) once daily w/ other antimycobacterials. Childn ≥6 mth Prevention of secondary case of group A streptococcal infection in penicillin allergic childn 12 mg/kg (max: 500 mg) once daily for 5 days. Mild to moderate typhoid 10 mg/kg once daily for 7 days. Infant & childn >6 mth 10 mg/kg once daily for 3 days, or initial dose of 10 mg/kg followed by 5 mg/kg daily for 4 days.

Administration

May be taken with or without food: May be taken w/ meals to reduce GI discomfort.

Contraindications

Hypersensitivity to azithromycin, erythromycin & any macrolide or ketolide antibiotic. History of cholestatic jaundice/hepatic dysfunction associated w/ prior use.

Special Precautions

Patients w/ pneumonia, cystic fibrosis, nosocomially acquired infections, known or suspected bacteremia, those requiring hospitalization, elderly or debilitated patients, significant underlying health problems that may compromise their ability to respond to their illness eg, immunodeficiency or functional asplenia. C. difficile-associated diarrhea. Discontinue if hepatitis occur. Patients at increased risk for prolonged cardiac repolarization. Exacerbation of myasthenia gravis symptoms. Hepatic impairment. Patients w/ GFR <10 mL/min. Lactation. Childn <6 mth.

Drug Interactions

Increased serum conc w/ nelfinavir. May potentiate effects of anticoagulants. Elevated digoxin conc. Acute ergot toxicity w/ ergotamine or dihydroergotamine. Terfenadine, cyclosporine, hexobarbital & phenytoin conc.
