Indications/Uses
4 mg: Chronic asthma, allergic rhinitis & prophylaxis for exercise-induced asthma. 5 mg: Add-on therapy for treatment of asthma in 6-14 yr patients w/ mild to moderate persistent asthma who are inadequately controlled on inhaled corticosteroids & in whom as-needed short-acting β-agonists provide inadequate clinical control. Alternative treatment option to low-dose inhaled corticosteroids for 6-14 yr patients w/ mild persistent asthma who do not have a recent history of serious asthma attacks that required oral corticosteroid use, & who have demonstrated that they are not capable of using inhaled corticosteroids. Prophylaxis of asthma from ≥6 yr in which the predominant component is exercise-induced bronchoconstriction.
Dosage/Direction for Use
Paed patient 6-14 yr 5 mg once daily in the evening, 2-5 yr 4 mg once daily in the evening.
Administration
May be taken with or without food: 5 mg: If taken w/ food, chew tab 1 hr before or 2 hr after food. 
Contraindications
Special Precautions
Do not use to treat acute asthma attacks. May present w/ systemic eosinophilia, sometimes presenting w/ clinical features of vasculitis consistent w/ Churg-Strauss syndrome. Do not substitute abruptly for inhaled or oral corticosteroids. Treatment does not alter the need for patients w/ aspirin-sensitive asthma to avoid taking aspirin & other NSAIDs. May be used during pregnancy & breast-feeding only if it is considered to be clearly essential. 5 mg: Periodically evaluate patients for asthma control. Not recommended as monotherapy in patients w/ moderate persistent asthma. No data on patients w/ severe hepatic impairment. Rare reports of drowsiness or dizziness. Not recommended in patients <6 yr.
Adverse Reactions
Rash. 4 mg: Headache, dizziness; abdominal pain, dyspepsia, infectious gastroenteritis; flu, cough, nasal congestion; fatigue, dental pain, fever, trauma. 5 mg: Upper resp infection. Diarrhoea, nausea, vomiting; elevated levels of serum transaminases (ALT, AST); pyrexia.
Drug Interactions
Caution particularly in childn, when co-administered w/ inducers of CYP3A4, 2C8, & 2C9 (eg, phenytoin, phenobarb & rifampicin). 5 mg: Decreased AUC w/ phenobarb. Increased systemic exposure w/ gemfibrozil & potential increase in adverse reactions w/ gemfibrozil or other potent inhibitors of CYP2C8.
