Indications/Uses

Skin & soft tissue infections caused by pneumococci; upper & lower resp tract infection.

Dosage/Direction for Use

Adult 250 mg IM every 4-6 hr or 500 mg IV over 3-4 min every 4-6 hr. Childn 2-10 yr ¹/₂ adult dose. Infant up to 2 yr ¹/₄ adult dose.

Hypersensitivity to penicillins & cephalosporins.

Special Precautions

Discontinue use if allergic reaction occurs. Serious & occasionally fatal hypersensitivity reactions eg, anaphylactoid & severe cutaneous adverse reactions; previous hypersensitivity reactions to penicillins, cephalosporins, carbapenems or other β-lactam agents. Patients w/ asthma, history of seizure disorder; diarrhoea; injury to blood brain barrier; inflamed meninges, during cardiopulmonary bypass; Candidiasis & other superinfections. Malnourished patients, low resistance to infection due to corticosteroids, immunosuppressants or irradiation. High-dose & long-term use. Renal impairment. Lactation. Premature & newborn infants. Elderly or debiliated patients.

Adverse Reactions

Pseudomembranous colitis; eosinophilia, agranulocytosis, leucopenia, thrombocytopenia, anaemia, thrombocytopenic purpura, neutropenia, mild SGOT elevation; anaphylactic & allergic reactions; thrombophlebitis (IV); nausea, diarrhoea, vomiting, epigastric discomfort, flatulence, loose stools; hepatic cholestasis; exanthema, urticaria; increased creatinine.

Drug Interactions

Higher serum peak conc & prolonged serum conc w/ probenecid. Possibility of skin rash w/ allopurinol. Prolonged &/or worsened diarrhea w/ antiperistaltic antidiarrheals eg, opiates, diphenoxylate, atropine. Interfered w/ bactericidal effects by chloramphenicol, erythromycin, sulphonamides, tetracyclines. Decreased effectiveness of OCs.
