Indications/Uses
Acute myelogenous & lymphoblastic leukaemia.
Dosage/Direction for Use
Induction therapy 60-200 mg/m2 daily.
Administration
Should be taken on an empty stomach.
Contraindications
Special Precautions
Discontinue use in patients w/ evidence of hepatic toxicity. Not recommended for maintenance therapy or similar long-term continuous therapy due to high risk of hepatic toxicity w/ vascular endothelial damage. Bone marrow suppression leading to leucopoenia & thrombocytopenia. Patients w/ inherited deficiency of thiopurine methyltransferase enzyme & NUDT15 gene variant; Lesch-Nyhan syndrome. Monitor full blood cell counts & hepatic function tests wkly during therapy. Asian patients. Limit exposure to sun- & UV light. Not to be taken by patients w/ galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Not recommended w/ live vaccines until at least 3 mth after completing chemotherapy. Combination w/ other immunosuppresive or chemotherapeutic agents. Hepatic or renal impairment. Avoid use during pregnancy & lactation.
Adverse Reactions
Bone marrow failure; venoocclusive hepatic disease: hyperbilirubinaemia, hepatomegaly, increased wt, portal HTN: splenomegaly, varices oesophageal, thrombocytopenia, increased hepatic enzyme, blood alkaline phosphatase & γ-glutamyl-transferase, jaundice, portal fibrosis, nodular regenerative hyperplasia, peliosis hepatitis. Stomatitis, GI disorder; venoocclusive hepatic disease in short-term cyclical therapy; hyperuricaemia, hyperuricosuria, urate nephropathy.
Drug Interactions
Not recommended w/ live vaccines. Risk of myelosuppression w/ other myelotoxic substances or RT. Concomitant use w/ aminosalicylate derivatives eg, olsalazine, mesalazine or sulphasalazine.
