Cholecalciferol (vit D3)
Indications/Uses
To fulfill the needs of vit D immediately during certain conditions eg, elderly, high risk/patient w/ infections or autoimmune diseases, pregnant & nursing mothers (1,000 IU only). 5,000 IU: Increase serum 25-hydroxycholecalciferol (25(OH)D) level in patients w/ vit D deficiency (serum levels of <30 ng/mL). 400 IU: Vit D3 supplementation.
Dosage/Direction for Use
Soft gelatin cap 1,000/5,000 IU: Adult & childn >12 yr 1 soft gelatin cap daily. May adjust the dose based on the 25-hydroxycholecalciferol (25(OH)D) serum levels or depending on the patient's needs & condition. Max: 5,000 IU daily. 2,000 IU: Adult Max: 4,000 IU daily until it reaches the optimum serum level (40-60 ng/mL), max duration: 30 days. Syr 400 IU: Adult, childn >6 yr, pregnant & nursing women 5 mL once daily. 1,000 IU: Adult & childn >12 yr 5 mL once daily.
Administration
Should be taken with food.
Contraindications
Hypersensitivity to vit D & its analogues. 1,000/5,000 IU: Not to be used in patients w/ hypercalcemia &/or hypercalciuria; nephrolithiasis or nephrocalcinosis; hypervitaminosis D. Severe renal impairment & renal failure.
Special Precautions
Discontinue if allergy symptoms occur. Ca consumption according to the recommended dietary allowance (RDA) to avoid hypercalcemia. Pregnant & nursing mothers. 1,000 IU: Check vit D blood level after ≥6 mth of use. 2,000 IU: Check vit D serum blood level after 3 mth of use. Risk for patients w/ kidney, liver, heart, lung disease; kidney stones; epilepsy & parathyroid hormone disorders. 5,000 IU: Not to be used in patients w/ tendency to renal calculi containing Ca; treatment of hyperparathyroidism associated w/ CV disease. Caution in patients receiving treatment for CV disease; suffering from sarcoidosis. Monitor Ca content in serum & urine. Patients w/ renal impairment; monitor Ca & phosphate levels. Consider risk of soft tissue calcification. Use other forms of vit D in patients w/ severe renal insufficiency.
Adverse Reactions
Pruritus, rash, urticaria. 1,000/5,000 IU: Hypercalcemia, hypercalciuria. 2,000/5,000 IU: Swollen face/tongue/lips, difficult to swallow, itching, difficulty in breathing. 1,000 IU: Hypersensitivity reactions, nausea, vomiting. 5,000 IU: Swollen throat.
Drug Interactions
Co-administration w/ phosphate infusion, cardiac glycoside, thiazide diuretic, vit D3 metabolites or vit D analogues, phenytoin, phenobarb, primidone, rifampicin, INH, orlistat, liqd paraffin, cholestyramine, glucocorticoid, multivit & mineral, calcitonin, etidronate, gallium nitrate, pamidronate, or plicamycin, Ca salt, conivaptan, aripiprazole, axitinib, saxagliptin, bile acid sequestrant, CYP3A4 inducer, deferasirox, mineral oil, sevelamer, tocilizumab. 5,000 IU: May interfere w/ vit D activity w/ cytotoxic agent actinomycin & imidazole anti-fungal agents. Increase levels/effects of Al hydroxide, Mg salts, sucralfate. Decrease levels/effects w/ corticosteroids (systemic), anti-convulsant, hydantoin, barbiturates.
