Spironolactone

Indications/Uses

CHF, nephrotic syndrome, ascites & oedema, malignant ascites, primary aldosteronism.

Dosage/Direction for Use

Adult 25-400 mg once daily, depending on the condition being treated. Childn & adolescent Number of tab will depend on wt. Elderly Low starting dose, then gradually increase as needed to obtain desired effect.

Overdosage

View Spirolon overdosage for action to be taken in the event of an overdose.

Administration

Should be taken with food.

Contraindications

Hypersensitivity. Concomitant use w/ K-sparing diuretics or any K supplements; eplerenone. Patients who cannot pass urine. Patients w/ hyperkalaemia; Addison's disease; severe kidney disease. Childn w/ moderate to severe kidney disease. Lactation.

Special Precautions

Risk of severe hyperkalaemia in patients w/ reduced kidney function or kidney failure & if co-administered w/ K supplements & food rich in K. May affect ability to drive or operate machinery. Caution in patients w/ kidney disease (especially childn w/ HTN or liver disease); difficulty passing urine; disease that can result in electrolyte balance disturbance in the blood (eg, K or Na); severe heart failure. Pregnancy. 25 mg: Contains lactose.

Adverse Reactions

Hyperkalaemia. Confusion; dizziness; vomiting & nausea; rash; itchy skin; muscle or leg cramps; kidney failure or abnormal function; breast enlargement & pain in men; malaise.

Drug Interactions

Potential interaction w/ digoxin or carbenoxolone; antihypertensive medicines including ACE inhibitors; other diuretics; NSAIDs eg, aspirin, indomethacin, mefenamic acid, ibuprofen; K supplements; antipyrine; heparin or LMWH; medicines known to cause hyperkalaemia; trimethoprim & trimethoprim-sulfamethoxazole; anesth; abiraterone. Reduced responsiveness to noradrenaline.
