Indications/Uses
Spiriva HandiHaler Maintenance treatment of COPD, associated dyspnoea, & for prevention of exacerbations. Spiriva Respimat Long-term maintenance treatment of bronchospasm & dyspnoea associated w/ COPD. Reduction of COPD exacerbations. As add-on maintenance bronchodilator treatment in patients ≥6 yr w/ severe asthma.
Dosage/Direction for Use
Spiriva HandiHaler 1 cap inhaled w/ HandiHaler once daily. Spiriva Respimat 2 puffs (5 mcg) once daily at the same time each day.
Contraindications
Hypersensitivity to tiotropium Br, atropine or its derivatives. Spiriva HandiHaler Hypersensitivity to the excipient lactose.
Special Precautions
Immediate hypersensitivity reactions may occur after administration. Inhaled medicines may cause inhalation-induced bronchospasm. Use w/ caution in patients w/ narrow-angle glaucoma, prostatic hyperplasia or bladder-neck obstruction; recent MI <6 mth; any unstable or life-threatening cardiac arrhythmia or cardiac arrhythmia requiring intervention or a change in drug therapy in the past yr; hospitalisation of heart failure (NYHA Class III or IV) w/in the past yr. Dry mouth, which has been observed w/ anticholinergic treatment, may in the long term be associated w/ dental caries. Should not be used more frequently than once daily. Occurrence of dizziness or blurred vision may influence ability to drive & use machinery. Should be used only if the expected benefit outweighs the potential risk in patients w/ moderate to severe renal impairment (CrCl ≤50 mL/min); closely monitor patients w/ moderate to severe renal impairment. Avoid contact w/ eyes. Pregnancy & lactation. Spiriva HandiHaler Not for the initial treatment of acute episodes of bronchospasm ie, rescue therapy. Spiriva Respimat Not for treatment of acute episodes of bronchospasm or for relief of acute symptoms. Should not be used as a 1st-line treatment for asthma.
Drug Interactions
Chronic administration w/ other anticholinergic drugs is not recommended.
