Paracetamol
Indications/Uses
Tab: Fever & mild to moderate pain (eg, headache, migraine). Relief of acute low back pain. Oral soln: Fever & mild to moderate pain (eg, headache, migraine, nerve pain, toothache, sore throat, period pains, & general aches & pains). Symptomatic relief of cold & flu. Supp: Pain & fever in childn.
Dosage/Direction for Use
Tab Adult & childn >12 yr 1-2 tab every 4-6 hr as required. Max dose: 8 tab in 24 hr. Childn 6-12 yr ½-1 tab every 4-6 hr when necessary. Max dose: 4 tab in 24 hr. Oral soln Always use measuring spoon (2.5 mL/5 mL) for accurate administration of dose. Leave at least 4 hr between doses. Childn 10-12 yr 20 mL, 4 times in 24 hr, 8-10 yr 15 mL, 4 times in 24 hr, 4-8 yr 10 mL, 4 times in 24 hr, 2-4 yr 7.5 mL, 4 times in 24 hr, 6-24 mth 5 mL, 4 times in 24 hr, 3-6 mth 2.5 mL, 4 times in 24 hr. Do not give >4 doses in any 24-hr period. Do not give for >3 days w/o medical advice. Infant 2-4 mth Post-vaccination fever One 2.5 mL spoonful, may be given up to 4 times daily starting at the time of vaccination. Do not give >4 doses in any 24-hr period. Seek medical advice if still needed 2 days after vaccination. Infant 2-3 mth weighing >4 kg & born after 37 wk Pain & other causes of fever One 2.5 mL spoonful. Give a 2nd dose after 4-6 hr if necessary. Do not give >2 doses. Seek medical advice if still feverish after 2 doses. Supp Do not exceed 4 times daily. Interval between doses should not be <4 hr. In case of renal or hepatic impairment, dose reduction or increased interval between administrations may be required. Childn 6-12 yr 1-2 supp (250-500 mg) each time, 1-5 yr 1-2 supp (125-250 mg) every 4-6 hr, up to a max of 4 doses in 24 hr. Infant 3-12 mth 1 supp (125 mg) every 4-6 hr, up to a max of 4 doses in 24 hr.
Administration
Tab: May be taken with or without food: Swallow whole or dissolve in water. Oral soln: May be taken with or without food.
Contraindications
Hypersensitivity. Severe liver disease.
Special Precautions
Administer w/ caution in patients w/ hepatic impairment or renal disease, acute hepatitis; patients taking enzymatic inducers &/or drugs that affect liver function; patients w/ history of alcoholism. Do not take w/ other paracetamol-containing medicines. Do not use for self-medication of high fever, fever lasting for a few days, or recurring fever. Avoid long-term treatment in patients w/ anemia, heart or lung disease. Tab: Consider risk of suffocation in administration to childn <12 yr. Oral soln: Do not give to infants <2 mth of age. Supp: Not recommended to use in infants <3 mth of age. Do not co-administer w/ any alcohol-containing medicines in childn.
Adverse Reactions
Neutropenia, thrombocytopenia, pancytopenia, leukopenia, agranulocytosis, skin rash, urticaria, dyspnoea, angioedema, nausea, vomiting, abdominal pain, hepatic insufficiency, asthma, hypotension.
Drug Interactions
Potential interaction w/ anticoagulants (eg, warfarin); antiepileptics/anticonvulsants; antibiotics (eg, rifampicin, chloramphenicol); HIV medicines (eg, zidovudine); cholestyramine; barbiturates; TCAs (eg, amitriptyline); domperidone, metoclopramide; OCs.
