Indications/Uses
Short-term treatment of moderate pain, especially following surgery, & of fever when IV administration is clinically justified by an urgent need to treat pain or hyperthermia &/or when other routes of administration are not possible.
Dosage/Direction for Use
Administer as 15-min IV infusion. 50-mL bottle Childn & toddler weighing >10 to ≤33 kg 15 mg/kg (1.5 mL/kg), max of 60 mg/kg daily not exceeding 2 g daily. Max vol per administration: 49.5 mL. 100-mL bottle Adult, adolescent & childn weighing >50 kg & no additional risk factors for hepatotoxicity 1 g (100 mL), max of 4 g daily. Max vol per administration: 100 mL; >50 kg w/ additional risk factors for hepatotoxicity 1 g (100 mL), max of 3 g daily. Max vol per administration: 100 mL; >33 to ≤50 kg 15 mg/kg (1.5 mL/kg), max of 60 mg/kg daily not exceeding 3 g daily. Max vol per administration: 75 mL. Min interval between each administration must be at least 4 hr. Patient w/ severe renal impairment (CrCl ≤30 mL/min) Reduce dose & increase min interval between each administration to 6 hr. Adult w/ hepatocellular insufficiency, chronic alcoholism, chronic malnutrition (low reserves of hepatic glutathione), dehydration Max daily dose: 3 g.
Contraindications
Hypersensitivity to paracetamol or propacetamol HCl (prodrug). Severe hepatocellular insufficiency.
Special Precautions
Prolonged or frequent use is discouraged. Risk of very serious liver damage in doses higher than those recommended. Use w/ caution in cases of hepatocellular insufficiency, severe renal insufficiency (CrCl ≤30 mL/min), chronic alcoholism, chronic malnutrition (low reserves of hepatic glutathione), dehydration, patients w/ genetically caused G6PD deficiency. Caution if concomitantly administered w/ flucloxacillin. If clinically needed during pregnancy, it should be used at the lowest effective dose for the shortest possible time & at the lowest possible frequency. No safety & efficacy data are available for premature newborn infants.
Adverse Reactions
Inj site reactions (pain & burning sensation). Rarely, hypotension, increased hepatic transaminases, malaise.
Drug Interactions
Almost 2-fold reduction in clearance w/ probenecid. Prolonged elimination t½ w/ salicylamide. Caution w/ concomitant intake of enzyme-inducing substances. Concomitant use of paracetamol (4,000 mg/day for at least 4 days) w/ oral anticoagulants may lead to slight variations of INR values. Concomitant use w/ flucloxacillin has been associated w/ increased risk of high anion gap metabolic acidosis, particularly in patients w/ severe renal impairment, sepsis, malnutrition & other sources of glutathione deficiency (eg, chronic alcoholism), as well as those using max daily doses of paracetamol.
