Indications/Uses
Treatment of the signs & symptoms of early-stage idiopathic Parkinson's disease as monotherapy or in combination w/ levodopa ie, over the course of the disease, through to late stages.
Dosage/Direction for Use
Leave patch on the skin for 24 hr, then ensure to take old patch off before putting new one on a different area of the skin each day. Change patches at about the same time every day. Monotherapy (early stage of Parkinson's disease) Initially one 2-mg patch daily. Increase by 2 mg wkly from the 2nd wk until maintenance dose is reached. Usual maintenance dose: 6-8 mg daily. Max dose: 8 mg daily. Combination w/ levodopa (advanced stage of Parkinson's disease) Initially one 4-mg patch daily. Increase by 2 mg wkly from the 2nd wk until maintenance dose is reached. Usual maintenance dose: 8-16 mg daily. Max dose: 16 mg daily.
Contraindications
Hypersensitivity. Patients undergoing MRI or cardioversion.
Special Precautions
Regularly check BP & eyesight during use. Gradually reduce or stop treatment to avoid dopamine agonist w/drawal syndrome or NMS. Avoid sunlight & solarium exposure on areas of skin showing any kind of skin reaction caused by the patch. Risk of dystonia or pleurothotonus (Pisa syndrome); changes in behaviour & abnormal thinking. Do not stick the patch on the same area of skin twice w/in 14 days. Do not stick the patch on broken, damaged, red, or irritated skin. Do not cut the patches into pieces. Contains Na metabisulphite (E223), which may rarely cause severe hypersensitivity reactions & bronchospasm. Patients who lose consciousness or feel dizzy should not drive or use machines. Caution in patients w/ serious liver problems. Do not use during pregnancy. Do not breast-feed during treatment. Do not give to childn <18 yr.
Adverse Reactions
Headache; feeling sleepy or dizzy; nausea, vomiting; skin reactions under the patch eg, redness & itching. Falling; hiccups; wt loss; swelling of legs & feet; feeling weak or tired; palpitation; constipation, dry mouth, heartburn; redness, increased sweating, itching; vertigo; hallucinations; low BP when standing up, high BP; difficulty falling asleep, sleep disorder, difficulty sleeping, nightmare, unusual dreams; dyskinesia; fainting, feeling dizzy when standing up because of fall in BP; unable to resist impulse to perform harmful action involving excessive gambling, repetitive meaningless actions, uncontrolled shopping or spending too much; binge eating, compulsive eating.
Drug Interactions
Decreased effect w/ antipsychotics & metoclopramide. Potential interaction w/ sedatives (eg, benzodiazepines), antidepressants, & alcohol. Worsened orthostatic hypotension w/ antihypertensives.
